2型糖尿病患者血尿酸与非酒精性脂肪肝疾病的相关性 被引量:8

Correlation between serum uric acid and nonalcoholic fatty liver disease in patients with type 2 diabetes mellitus

摘要 目的探讨2型糖尿病(T2DM)患者中血尿酸(SUA)水平与非酒精性脂肪肝病(NAFLD)的关系。方法对合肥市第一人民医院2010年1月至2016年12月收治的1808例T2DM患者进行横断面研究,通过腹部B超诊断NAFLD,分为非NAFLD组和NAFLD组,其中NAFLD组患者668例,非NAFLD组患者1140例。按照SUA四分位数分为4组(Q1~Q4),分析SUA水平与代谢指标之间的关系。结果本研究人群中NAFLD的发生率为36.9%。Q1、Q2、Q3和Q4四组间年龄、病程、体质指数(BMI)、收缩压(SBP)、舒张压(DBP)、三酰甘油(TG)、丙氨酸氨基转移酶(ALT)、天冬氨酸氨甲酰转移酶(AST)、γ-谷氨酰转肽酶(γ-GT)、血清肌酐(SCr)、空腹血糖(FPG)、空腹C肽(FCP)、高密度脂蛋白胆固醇(HDL-C)、总胆红素(TBIL)差异均具有统计学意义(P<0.05),总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)差异无统计学意义(P>0.05)。在男性中NAFLD的患病率随SUA水平的逐步升高(Q1、Q2、Q3、Q4)逐渐增加(13.07%,32.50%,41.71%,49.75%,P<0.01)。Spearman相关分析显示了SUA与其他代谢指标之间的相关性,在两种性别中SUA水平与BMI、SBP、TG、ALT、AST、γ-GT、SCr、FCP均显著正相关(P<0.01),与FPG、HDL-C、TBIL呈负相关(P<0.01,P<0.05)。多元Logistic回归分析结果显示,BMI、TG、ALT、FCP、SUA是NAFLD的影响因素(P<0.01,P<0.05)。结论T2DM男性患者中SUA与NAFLD密切相关,是NAFLD的独立危险因素。 Objective To investigate the relationship between serum uric acid(SUA)level and nonalcoholic fatty liver disease(NAFLD)in patients with type 2 diabetes mellitus(T2 DM).Methods A cross-sectional study was conducted in 1808 patients with T2 DM from January 2010 to December 2016.By abdominal ultrasound diagnosis,there were 668 patients in NAFLD group and 1140 patients in non NAFLD group.According to the quartile of SUA level,the patients were divided into four groups(Q1-Q4),and the relationship between SUA level and metabolic indicators was analyzed.Results The incidence of NAFLD was 36.9%.There were significant differences in age,disease duration,body mass index(BMI),systolic blood pressure(SBP),diastolic blood pressure(DBP),triglyceride(TG),alanine aminotransferase(ALT),aspartate aminotransferase(AST),γ-glutamyl transpeptidase(γ-GT),serum creatinine(SCr),fasting plasma glucose(FPG),fasting C-peptide(FCP),high density liptein cholesterol(HDL-C)and total bilirubin(TBIL)(P<0.05),while there was no significant difference in total cholesterol(TC)and low-density lipoprotein cholesterol(LDL-C)(P>0.05)among the Q1,Q2,Q3 and Q4 group.The prevalence of NAFLD in men was 13.07%,32.50%,41.71%and 49.75%in group Q1,Q2,Q3 and Q4,respectively(P<0.01).Spearman correlation analysis showed SUA level was significantly positively correlated with BMI,SBP,TG,ALT,AST,γ-GT,SCr and FCP(P<0.01)and was negatively correlated with FPG,HDL-C and TBIL in both genders(P<0.05).Multiple Logistic regression analysis revealed that BMI,TG,ALT,FCP and SUA were the influencing factors for NAFLD(P<0.05).Conclusion SUA is closely related to NAFLD in male patients with T2 DM and is an independent risk factor for NAFLD.
