摘要
目的分析不同手术体位下经皮肾镜碎石术对复杂性肾结石的医治效果。方法抽选2019年1月—2020年1月期间于该院以经皮肾镜碎石术医治复杂性肾结石的60例患者,以其采取的不同手术体位为分组依据,将其分为采取完全俯卧体位的对照组、采取截石斜仰卧体位的研究组,每组30例。组间对比术中出血量、住院时间、手术时间及结石清除率、并发症发生率。结果研究组的住院时间、手术时间比对照组短,术中出血量比对照组少,差异有统计学意义(P<0.05)。研究组的结石清除率、并发症发生率为90.00%和6.67%,与对照组的80.00%和13.33%相近,差异无统计学意义(χ^(2)=0.523、0.185,P=0.470、0.667>0.05)。结论不同手术体位下经皮肾镜碎石术对复杂性肾结石均有显著治疗效果和安全性,但截石斜仰卧体位可减少出血、缩短手术时间,加快患者康复,因此临床应为患者采取这种手术体位。
Objective To analyze the therapeutic effects of percutaneous nephrolithotomy in different surgical positions for complicated kidney stones.Methods Select 60 patients with complicated kidney stones treated by percutaneous nephrolithotomy in the hospital from January 2019 to January 2020,and divide them into groups based on their different surgical positions:the control group took the prone position and the study group took the stone-cut oblique supine position,with 30 cases in each group.The intraoperative blood loss,hospitalization time,operation time,stone removal rate,and complication rate were compared between groups.Results The hospitalization time and operation time of the study group were shorter than those of the control group,and the intraoperative blood loss was less than that of the control group.The differences were statistically significant(P<0.05).The stone removal rate and complication rate of the study group were 90.00%and 6.67%,which were similar to 80.00%and 13.33%of the control group,and the difference was not statistically significant(χ^(2)=0.523,0.185,P=0.470,0.667>0.05).Conclusion Percutaneous nephrolithotomy under different surgical positions has significant therapeutic effect and safety for complex kidney stones,but the oblique supine position for lithotomy can reduce bleeding,shorten the operation time,and speed up the patient's recovery.Therefore,it should take this surgical position in the clinic for patients.
