摘要
目的研究在儿童呼吸道感染诊断中进行全血C-反应蛋白(CRP)联合白细胞计数(WBC)测定的价值。方法选择2018年1月—2019年12月120例呼吸道感染患儿为对象,按照感染类型分为两组,其中呼吸道病毒性感染患儿有60例,呼吸道细菌性感染患儿有60例,均测定CRP、WBC,比较结果差异,并分析指标的诊断价值。结果细菌性感染组CRP、WBC水平为均高于病毒性感染组(20.52±3.29)mg/L vs(6.84±1.43)mg/L、(18.57±1.08)×10^(9)/L vs(7.48±2.19)×10^(9)/L,差异有统计学意义(t=29.539、35.180,P<0.05);细菌性感染组患儿治疗后CRP、WBC水平均低于治疗前,差异有统计学意义(P<0.05);CRP联合WBC用于诊断呼吸道细菌性感染的敏感度为90.00%,特异度为96.67%,准确度为93.33%,阳性预测值为96.43%,阴性预测值为90.63%。结论小儿呼吸道细菌性感染CRP、WBC水平较呼吸道病毒性感染有更明显的异常升高,通过测定CRP、WBC水平可辅助诊断、鉴别呼吸道感染,两者联合检测的诊断效能较高。
Objective To study the value of measuring C-reactive protein(CRP)combined with white blood cell count(WBC)in the diagnosis of respiratory tract infections in children.Methods From January 2018 to December 2019,120 children with respiratory tract infection were selected as subjects,and they were divided into 2 groups according to the type of infection.Among them,there were 60 children with respiratory viral infection and 60 children with respiratory bacterial infection.Both CRP and WBC were measured,the results were compared,and the diagnostic value of the indicators was analyzed.Results The levels of CRP and WBC in the bacterial infection group were higher than those in the viral infection group(20.52±3.29)mg/L vs(6.84±1.43)mg/L,(18.57±1.08)×10^(9)/L vs(7.48±2.19)×10^(9)/L,the difference was statistically significant(t=29.539,35.180,P<0.05);the levels of CRP and WBC after treatment in children in the bacterial infection group were lower than before treatment,the difference was statistically significant(P<0.05);CRP combined with WBC was used to diagnose respiratory bacteria sensitivity of infection was 90.00%,the specificity was 96.67%,the accuracy was 93.33%,the positive predictive value was 96.43%,and the negative predictive value was 90.63%.Conclusion The levels of CRP and WBC in bacterial respiratory infections in children are more abnormally elevated than viral infections in the respiratory tract.The determination of CRP and WBC levels can assist in the diagnosis and differentiation of respiratory tract infections.The combined detection of the two has a higher diagnostic efficiency.
