摘要
目的探讨帕金森病前驱期(pPD)轻度认知损害(MCI)的特点,并进行MCI与其他临床特征的相关性分析。方法收集32例p PD患者(p PD组),按国际运动障碍协会制定的MCIⅡ级评估标准分为:p PD-MCI组(20例)和不符合MCI诊断标准的12例为pPD-NC组,另选择正常者23例为对照组。完成人口学资料、临床症状量表和成套神经心理评估测验等资料的收集,比较3组临床资料,分析pPD认知功能与其他临床症状的相关性。结果两组的认知域z分数比较,pPD组的整体认知、注意/工作记忆、执行功能、记忆、视空间和言语z分数均低于对照组,差异均有统计学意义(P=0.05)。p PD组最常见的认知损害子域为:记忆域(56.25%),其次为执行功能(43.75%)、注意/工作记忆(40.63%)。相关性分析:整体认知z分数与汉密尔顿抑郁量表评分(r=-0.3975,P=0.0243)、UPDRSⅢ(r=-0.4949,P=0.004)呈负相关,与受教育年限呈正相关(r=0.4651,P=0.0073)。结论p PD患者合并MCI,记忆、执行功能、注意/工作记忆是常见受损认知域,且认知功能与抑郁、运动症状及受教育年限低密切相关,应重视前驱期MCI的早期识别和干预,改善预后。
Aim To investigate the characteristics of mild cognitive impairment(MCI)in prodromal Parkinson’s disease(pPD),and analyze the correlation between cognition and other clinical features.Methods Three groups of participants were recruited in this study,including pPD with MCI group(pPD-MCI,20 cases),pPD patients with normal cognition group(pPD-NC,12 cases),and healthy control group(HC,23 cases).General demographic data and clinical symptoms assessments were collected from all the subjects.Besides,all participants underwent a formal,comprehensive neuropsychological battery to assess cognition so that the relationship between other clinical symptoms and cognitive performance could be elucidated.Results Compared to the HC group,the p PD group exhibited poorer z score of global cognition,attention and working memory,executive function,memory,visuospatial function and language.Furthermore,the global cognitive z score of p PD patients were negatively related to the Hamilton Rating Scale for Depression(HAMD)and Part III of the Unified Parkinson’s Disease Rating Scale(UPDRS III),but positively related to the education.Conclusion The high frequency of MCI in p PD patients,the most frequent domain impaired was memory,and which were followed by executive function and attention and working memory.In addition,cognitive function is closely related to lower education,severe depression and motor symptoms,which suggests that attention should be paid to the early recognition of MCI in prodromal stage,so as to provide early intervention.
