摘要
目的探讨RT-Mind软件在乳腺癌临床靶区(Clinical Target Volumes,CTV)及危及器官(Organs-At-Risk,OARs)自动勾画的可行性,为临床应用提供依据。方法选取10例乳腺癌保乳术后放疗患者,由临床医生手动勾画CTV及OARs(左肺、右肺、心脏及健侧乳腺),并以此勾画结果作为参考标准,再使用RT-Mind软件自动勾画CTV及OARs。比较手动及自动勾画的相似性系数(DSC)、Jaccard系数(JAC)、Hausdroff距离(HD)及体积差异(VD)等参数以评估勾画效果。对于自动勾画的CTV,沿Body内收0.5 cm作为新的自动勾画临床靶区CTV_(内收0.5 cm),再通过上述评估参数评估其勾画效果。结果CTV自动勾画的DSC值、JAC值、VD值、HD值分别为0.77±0.06、0.64±0.07、(33.47±18.66)%、(2.53±0.64)cm;CTV_(内收0.5 cm)的DSC值、JAC值、VD值、HD值分别为0.83±0.05、0.71±0.07、(17.12±12.11)%、(2.53±0.64)cm,相比自动勾画CTV,除HD值无变化外,其它评价参数均有明显改善。四个OARs的HD均值均小于2.00 cm,四个OARs的DSC、JAC、VD均值比较:双肺>心脏>健侧乳腺,表明双肺勾画效果最好,心脏次之,最后是健侧乳腺。结论RT-Mind软件能够较为准确的实现乳腺癌患者OARs的自动勾画。对于乳腺癌CTV,RT-Mind软件自动勾画效果不理想,而将自动勾画的CTV沿Body内收0.5 cm后,其勾画效果有明显提高。RT-Mind软件能够为临床医生节省时间,提高工作效率。
Objective To explore the feasibility of RT-Mind software for the automatic delineation of clinical target volumes(CTV)and organs-at risk(OARs)in the radiotherapy for breast cancer so as to provide a basis for the clinical application.Methods A total of 10 patients with breast cancer after breast-conserving surgery were delineated for the CTV and OARs(L-lung,R-lung,heart,healthy breast)by an experienced radiation oncologist,and the contouring results were defined as the reference standard.And then the CTV and OARs were automatic delineation by RT-Mind software.Several evaluation parameters such as Dice coefficient(DSC),Jaccard coefficient(JAC),Hausdroff distance(HD),and volume difference(VD)were used to evaluate the contouring results.Results The DSC,JAC,VD,HD values of automated delineation of CTV were 0.77±0.06,0.64±0.07,(33.47±18.66)%,(2.53±0.64)cm,respectively.The DSC,JAC,VD,HD values of automated delineation of CTV which retracted along the Body by 0.5 cm(CTV_(retracted 0.5 cm))were 0.83±0.05,0.71±0.07,(17.12±12.11)%,(2.53±0.64)cm,respectively.Compared with automated delineation CTV,CTV_(retracted 0.5 cm) can significantly improve the evaluation parameters,except for the value of HD.The HD values of four organs-at risk were less than 2.00 cm.Comparison of DSC,JAC,and VD values of four OARs:lungs>heart>contralateral breast,indicating the effect of automatic delineation were lungs>heart>contralateral breast.Conclusion RT-Mind software can accurately realize the automatic delineation of OARs in radiotherapy for breast cancer.For CTV in breast caner,the effect of automatic delieating of RT-Mind software is not ideal.When the CTV is retracted along the Body by 0.5 cm,the effect of automatic delieating for CTV can significantly improved.RT-Mind software can save time and improve the working efficiency for clinical doctors.
