NSE、TTF-1及Ki-67与小细胞肺癌患者预后、化疗反应的关系 被引量:7

Relationship between NSE,TTF-1,Ki-67 and the prognosis and chemotherapy response in patients with small cell lung cancer

摘要 目的探讨神经元特异性烯醇化(NSE)、甲状腺转录因子-1(TTF-1)及Ki-67在小细胞肺癌组织中的表达水平,并分析三者与小细胞肺癌患者预后及化疗反应的关系。方法选取2015年8月至2018年8月本院收治的90例小细胞癌患者,观察血清NSE、癌组织中Ki-67、TTF-1表达情况,探讨三者与预后的关系,分析三个指标表达情况对化疗反应率的影响。结果小细胞肺癌组织中Ki-67及TTF-1阳性率、NSE表达异常增高率78.89%、76.67%、75.56%。90例小细胞肺癌患者2年生存率为24.44%(22/90),TTF-1(阳性)与NSE(高表达)、Ki-67(阳性)表达的小细胞肺癌患者死亡率更高。临床分期(广泛期)、有淋巴结转移、TTF-1表达(阳性)、NSE(高表达)及Ki-67表达(阳性)为影响小细胞肺癌患者预后生存的独立危险因素(P<0.05)。NSE正常表达一线化疗疾病控制率(DCR)、客观反应率(ORR)均低于NSE异常增高患者,差异有统计学意义(P<0.05);TTF-1阳性及Ki-67阳性患者一线化疗DCR、ORR均高于TTF-1及Ki-67阴性表达的患者,差异有统计学意义(P<0.05)。NSE(异常增高)、TTF-1(阳性表达)及Ki-67(阳性表达)的患者PFS、OS均显著短于NSE(正常表达)、TTF-1(阴性表达)、Ki-67(阴性表达)者,差异具有统计学意义(P<0.05)。结论 TTF-1、NSE及Ki-67对小细胞肺癌的发生及化疗反应起重要作用,在临床工作中可通过联合检查三者的表达情况,从而选择有效的个体化治疗方案,以延长患者生存期。 Objective To investigate the expression of neuron specific enolization(NSE),Thyroid transcription factor-1(TTF-1)and Ki-67 in small cell lung cancer and to analyze the relationship between three indicators and the prognosis and chemotherapy response in patients with small cell lung cancer. Methods The 90 patients with small cell carcinoma admitted to our hospital from August 2015 to August 2018 were selected to observe the expression of serum NSE and Ki-67 and TTF-1 in cancer tissues and explore the correlation with the prognosis. The influence of the expression of three indicators on the response rate for chemotherapy was also analyzed. Results The positive rates of Ki-67 and TTF-1 and the abnormal increasing rate of NSE expression in small cell lung cancer tissues were 78.89%,76.67%,and 75.56%,respectively(P<0.05). The 2-year survival rate of 90 patients with small cell lung cancer was 24.44%(22/90),and the mortality of patients with TTF-1(positive),NSE(high expression)and Ki-67(positive)expression was higher.Clinical stage(extensive stage),lymph node metastasis,TTF-1 expression(positive),NSE(high expression)and Ki-67 expression(positive)were independent risk factors affecting the prognosis survival of patients with small cell lung cancer(P<0.05). The DCR and ORR of the first-line chemotherapy of patients with normal expression of NSE were lower than those of patients with abnormal increased NSE(P<0.05). The DCR and ORR of patients with positive TTF-1 and Ki-67 were higher than those with negative expression of TTF-1 and Ki-67(P<0.05). PFS and OS of patients with NSE(abnormal increase),TTF-1(positive expression)and Ki-67(positive expression)were significantly shorter than those of patients with NSE(normal expression),TTF-1(negative expression),Ki-67(negative expression)(P<0.05). Conclusion TTF-1,NSE and Ki-67 play an important role in the occurrence of small cell lung cancer and chemotherapy response. In clinical work,the expression of the three can be combined to selecteffective individualized treatment plans to prolong patient survival.
