摘要
目的探讨血清25羟维生素D[25-(OH)D]与2型糖尿病(T2DM)男性患者勃起功能障碍(ED)的相关性。方法选取2017年5月~2019年10月我院收治的T2DM合并ED患者103例为观察组,单纯T2DM患者104例为T2DM组,单纯ED患者81例为ED组,同期体检健康的78例健康男性为对照组。根据血清中25-(OH)D水平将观察组分为A组[34例,25-(OH)D>30 ng/ml]和B组[69例,25-(OH)D≤30 ng/ml],再按照随机数字法将B组分为B1组34例和B2组35例。A组及B1组患者采用口服降糖药物联合胰岛素治疗。B2组患者在常规治疗基础上加服0.25μg维生素D制剂。比较各组受试者的血清糖化血红蛋白(HbA1c)、空腹血糖(FPG)、血脂、血清25-(OH)D、性激素水平及国际勃起功能问卷表(IIEF)评分,分析25-(OH)D与T2DM合并ED的关系。结果4组受试者FPG、HbA1c、低密度脂蛋白胆固醇(LDL-C)、总胆固醇(TC)、25-(OH)D、睾酮(T)、卵泡刺激素(FSH)、黄体生成素(LH)水平、IIEF评分比较差异均有统计学意义(P<0.01);其中,观察组患者FPG、25-(OH)D、T水平及IIEF评分均低于T2DM组、ED组及对照组(P<0.01);观察组患者FSH及LH水平高于T2DM组、ED组及对照组(P<0.05)。治疗后,B1组患者25-(OH)D、T水平及IIEF评分均低于B2及A组(P<0.05);A组患者FSH及LH水平低于B1及B2组(P<0.05)。Pearson相关分析结果显示,男性T2DM合并ED患者25-(OH)D与T、IIEF评分呈正相关,与FSH、LH呈负相关(P<0.05)。结论血清25-(OH)D水平与T2DM合并ED的发生呈明显正相关,补充维生素D可明显改善患者性激素水平,促进ED患者功能恢复,具有临床应用价值。
Objective To investigate the correlation between serum 25-hydroxyvitamin D[25-(OH)D]and erectile dysfunction(ED)in male patients with type 2 diabetes mellitus(T2DM).Methods Select 103 patients with T2DM and ED in our hospital from May 2017 to October 2019 as the observation group,104 patients only with T2DM as the T2DM group,81 patients only with ED as the control group,and 78 healthy men with healthy physical examination in the same period as the control group.Then the observation group was divided into group A[34 cases,25-(OH)D>30 ng/ml]and group B[69 cases,25-(OH)D≤30 ng/ml]according to serum 25-(OH)D level.Then the group B was divided into the group B1(34 cases)and group B2(35 cases).Groups A and B1 were treated with oral hypoglycemic drugs combined with insulin.In the group B2,0.25μg vitamin D preparation was added to the conventional treatment.The levels of serum glycosylated hemoglobin(HbA1c),fasting plasma glucose(FPG),blood lipid,serum 25-(OH)D,sex hormone and International Erectile Function Questionnaire(IIEF)scores in each group were compared and the relationship between 25-(OH)D and T2DM combined ED was analyzed.Results FPG,HbA1c,low density lipoprotein cholesterol(LDL-C),total cholesterol(TC),25-(OH)D,testosterone(T),follicle stimulating hormone(FSH),luteinizing hormone(LH)levels and IIEF score in 4 groups showed statistically significant differences(P<0.01).Among them,FPG,25-(OH)D,T level and IIEF scores in the observation group were lower than those in T2DM group,ED group and control group(P<0.01).FSH and LH levels in the observation group were higher than those in T2DM group,ED group and control group(P<0.05).After treatment,25-(OH)D,T,levels and IIEF scores in group B1 were lower than those in group B2 and A(P<0.05).FSH and LH levels were lower than those in group B1 and B2(P<0.05).Pearson correlation analysis result showed that 25-(OH)D level of male T2DM combined ED patients was positively correlated with T and IIEF score,while negatively correlated with FSH and LH(P<0.05).Conclusion Serum 25-(OH)D level is positively correlated with the occurrence of T2DM with ED.The treatment of this disease with vitamin D can significantly improve the sex hormone level and promote the functional recovery of ED patients,which has clinical application value.
