摘要 目的:采用数据挖掘技术探索康艾注射液不良反应(ADR)发生特点与影响因素的关联性,为临床合理、安全使用中药注射剂提供参考。方法:收集近5年康艾注射液ADR文献案例和国家ADR监测系统中某两家医院上报数据,以IBM SPSS Modeler为平台,采用决策树(CHAID)算法、贝叶斯网络和关联规则Apriori算法,综合开展数据挖掘研究。结果:CHAID算法显示用药剂量是关键影响因素,40 mL和50 mL发生ADR有52例,占比最高(48.148%),其中全身性损害最多,有17例(构成比32.692%),其次是皮肤及附件损害,有15例(构成比28.846%)。贝叶斯网络分析显示,用药剂量是最重要的预测因子,概率为42.27%。关联规则Apriori算法中置信度大于80%的前两条规则显示,无过敏史的女性患者在连续用药后,发生ADR案例为11例,占全部案例10.19%,其中90.91%的案例表现为皮肤及附件损害。有过敏史且用药剂量为40 mL的患者发生ADR案例为11例,占全部案例10.19%,其中81.82%的案例表现为全身性损害。结论:三种挖掘方法结果较为一致,康艾注射液ADR发生可能与用药剂量、患者年龄、性别、溶媒类型、溶媒用量、过敏史等因素相关,其发生规律尚需大样本量数据进一步分析验证。 Objective:Using data mining technology to study the ADR characteristics of Kangai injection and relevant factor,to provide reference for rational and safe use of TCM injections in clinic.Methods:The ADR literature cases of Kangai injection and the data reported by two hospitals in the national ADR monitoring system in recent 5 years were collected,based on IBM SPSS Modeler platform,adopting decision tree(CHAID)algorithm,Bayes network and Association rule Apriori algorithm,combining these ways to carry out data mining research.Results:CHAID algorithm indicates dosage is the key factor.There were 52 ADR cases occurred with dosage of 40 mL and 50 mL,48.148%is the highest proportion,there exists 17 cases of systemic damages,the number is the most,ratio is 32.692%,the second were 15 cases of lesions of skin and its appendages,proportion is 28.846%.Bayes network analysis reveals that dosage is the most important predictive factor,the probability is 42.27%.With association rule Apriori algorithm,rules with confidence over 80%shows that there are 11 ADR cases occurred in female patients with no allergic history after successive taking the medicine,the ratio is 10.19%,the inside proportions of lesions of skin and appendages is 90.91%.There are 11 ADR cases among the patients with allergic history and 40 mL dosage,the ratio is 10.19%,the inside proportions of systemic damages is 81.82%.Conclusion:The results of three kinds of analyze are basically the same,the ADR of Kangai injection is probably relevant with dosage,patients'age,gender,allergic history,type and quantity of solvent,it needs moresample data to promote analyzing and verifying occurrence regularity of ADR.
