摘要
目的:对比分析腹部X线平片与多层螺旋CT对肠梗阻患者的诊断价值。方法:选取2018年3月至2020年9月临床上收治的70例疑似肠梗阻患者,均给予腹部X线平片及多层螺旋CT检查,比较两种方法诊断肠梗阻结果、诊断效能及舒适度、图像清晰度及操作难度评分。结果:手术确诊肠梗阻患者54例,多层螺旋CT诊断肠梗阻准确率为85.19%,腹部X线平片诊断肠梗阻准确率为66.67%,多层螺旋CT诊断肠梗阻准确率较腹部X线平片显著提高(P<0.05);以手术病理诊断作为金标准,多层螺旋CT诊断肠梗阻的灵敏度、特异度、阳性预测值、阴性预测值及Kappa值均较腹部X线平片显著提高(P<0.05);多层螺旋CT诊断肠梗阻舒适度、图像清晰度评分较腹部X线平片显著提高(P<0.05),操作难度评分较腹部X线平片显著降低(P<0.05)。结论:与腹部X线平片相比,多层螺旋CT诊断肠梗阻准确性更高,能够反映肠梗阻的类型及原因,并且患者舒适度和图像清晰度高,操作难度低,值得临床推广应用。
Objective:To compare and analyze the diagnostic value of abdominal X-ray plain film and multi-slice spiral CT in patients with intestinal obstruction.Methods:Select 70 patients with suspected intestinal obstruction who were clinically admitted from March 2018 to December 2020,and all were given abdominal X Line plain film and multi-slice spiral CT examination;compare the two methods in the diagnosis of intestinal obstruction,diagnostic efficiency and comfort,image clarity,and operation difficulty score.Results:54 patients with intestinal obstruction were diagnosed by surgery,and multi-slice spiral CT in diagnosis of intestinal obstruction.The accuracy rate was 85.19%,and the accuracy rate of abdominal X-ray plain film in diagnosing intestinal obstruction was 66.67%.The accuracy of multi-slice spiral CT in diagnosing intestinal obstruction was significantly higher than that of abdominal X-ray film(P<0.05).The surgical pathological diagnosis was taken as the gold standard.The sensitivity,specificity,positive predictive value,negative predictive value and Kappa value of multi-slice spiral CT in the diagnosis of intestinal obstruction are compared with plain abdominal X-ray film(P<0.05).Multi-slice spiral CT diagnosis of intestinal obstruction comfort and image clarity score.Compared with plain abdominal X-ray film,the difficulty of operation score is significantly lower than that of plain abdominal X-ray film(P<0.05).Conclusion:Compared with plain abdominal X-ray film,multi-slice spiral CT is more accurate in diagnosing intestinal obstruction and reflects the type and cause of intestinal obstruction,and the patient comfort and image clarity are high,and the operation difficulty is low.It is worthy of promotion.
