摘要
目的:介绍李岩教授中西医保守治疗异位妊娠的经验,扶正祛邪法的运用及用药时机的确立是获得良好治疗结局的关键点及创新点。方法:通过中西医结合[消癥杀胚煎剂辅助甲氨碟呤(Methotrexatc,MTX)、米非司酮(Mifepristone,RU486)]治疗、临床观察,并结合典型病例。治疗过程中密切注意患者病情的变化,随症加减、随病情变化加减用药是中医药治疗本病的优势体现。结果:探讨异位妊娠中西医结合保守治疗的新思路。扶正不留瘀,祛瘀不伤正,瘀血贯穿了异位妊娠的形成、发展过程。未破损期异位妊娠的治疗,既要杀胚与消癥并举,同时也要不拘于古方,不墨守陈规。结论:异位妊娠未破损期使用消癥杀胚煎剂辅助甲氨碟呤(MTX)、米非司酮(RU486)保守治疗效果较好,具有手术及纯西药治疗不可替代的优势。临床上在与患者充分沟通的前提下评估患者病情,可优选此种方法。
Objective:It's to introduce Professor Li Yan's experience in the conservative treatment of ectopic pregnancy.The application of fuzheng exorcism and the establishment of medication timing are the key points and innovations to achieve a good therapeutic outcome.Methods:Combined traditional Chinese and western medicine[Xiaozheng Embryo Killing decoction assisted with MTX and Mifepristone(RU486)]was used for treatment and clinical observation,and typical cases were combined.During the treatment,we should pay close attention to the changes of patients'condition.The advantages of traditional Chinese medicine(TCM)in the treatment of this disease are reflected in the addition and subtraction of drugs according to the syndromes and the changes of conditions.Results:It's to explore the new idea of conservative treatment of ectopic pregnancy.The stasis runs through the formation and development process of ectopic pregnancy.The treatment of ectopic pregnancy in undamaged period should not only kill the embryo and eliminate the disease simultaneously,but also pay close attention to the changes of the condition.Don't stick to the old rules.Conclusion:In the undamaged period of ectopic pregnancy,the use of Xiaozheng Embryo Killing decoction agent assisted with methotrexate(MTX)and mifepristone(RU486)has a better conservative treatment effect,which has irreplaceable advantages in surgery and pure western medicine treatment.This method can be optimized for clinical evaluation of the patient's condition under the premise of full communication with the patient.
