多参数皮下组织超声检查鉴别乳腺良恶性肿瘤的诊断分析

摘要 目的:分析多参数皮下组织超声检查在乳腺良恶性肿瘤鉴别诊断中的应用价值。方法:挑选2019年5月—2020年5月在我院收治的160例乳腺肿块患者,经超声检查获取二维形态、血流Adler分级等超声参数及超声弹性成像,分析比较面积比法、应变率方法和Itoh法在诊断乳腺肿瘤良恶性方面的应用价值,并且以病理诊断结果作为金标准,分析三种方法对乳腺肿瘤良恶性鉴别诊断的应用价值,构建回归模型并分析不同因素诊断乳腺恶性肿瘤的具体临床价值。结果:良性病灶与恶性病灶的长径和面积在弹性成像和灰阶成像中差异明显(P<0.05);恶性病灶的长径和面积均大于良性肿瘤(P<0.05);在敏感度、特异度、准确度、阳性预测率及阴性预测率方面,面积比法和应变率方法均高于I t o h法(P<0.05);面积比法和应变率方法比较无明显差异(P>0.05)。L o g i s t i c回归模型最终入选6个因素(均P<0.05)。结论:针对乳腺良恶性肿瘤的鉴别诊断,弹性成像更具有优越性,面积比法和应变率方法较Itoh法而言,在诊断敏感性、特异性、准确性等方面的临床应用,分别使用乳腺肿瘤超声参数纵横比、微钙化、弹性评分等多个指标构建回归模型,其对于诊断乳腺癌具有十分重要的意义,能够提升疾病诊断的准确性。 Objective To analyze the application value of multi-parameter subcutaneous ultrasound in the differential diagnosis of breast benign and malignant tumors.Methods Select 160 patients with breast masses who were admitted to our hospital from May 2019 to May 2020.Obtain two-dimensional morphology,blood flow Adler classification and other ultrasound parameters and ultrasound elastography through ultrasound examination,and analyze and compare the area ratio method and strain.The application value of rate method and Itoh method in the diagnosis of benign and malignant breast tumors,and the pathological diagnosis results as the gold standard,analyze the application value of the three methods in the differential diagnosis of benign and malignant breast tumors,construct a regression model and analyze different factors to diagnose malignant breast.The specific clinical value of tumors.Results The long diameter and area of benign lesions and malignant lesions were significantly different in elastography and grayscale imaging(P<0.05);the long diameter and area of malignant lesions were larger than those of benign tumors(P<0.05);in sensitivity and specificity In terms of degree,accuracy,positive prediction rate and negative prediction rate,the area ratio method and the strain rate method are higher than the Itoh method(P<0.05);there is no significant difference between the area ratio method and the strain rate method(P>0.05).The logistic regression model finally selected 6 factors(all P<0.05).Conclusion For the differential diagnosis of benign and malignant breast tumors,elastography is more advantageous.Compared with the Itoh method,the area ratio method and strain rate method are used in clinical applications in terms of diagnostic sensitivity,specificity,and accuracy.The regression model is constructed by multiple indicators such as the aspect ratio of tumor ultrasound parameters,microcalcification,and elasticity score,which is of great significance for the diagnosis of breast cancer and can improve the accuracy of disease diagnosis.
