摘要
二分髌骨(bipartite patella,BP)是临床上一种相对少见的膝关节疾病,由于通常无症状,常常被患者以及骨科医生忽视。随着目前对二分髌骨的研究逐渐明了,其发病机制主要是由于在发育过程中骨化中心不融合导致髌骨形成两个不发生骨性联合的骨块所引起。目前诊断方法有膝关节X线片、CT及MRI。其中MRI被视为诊断二分髌骨的金标准。治疗方法主要有保守治疗、副髌骨切除术、软组织松解术和副髌骨复位内固定术等。各种治疗方式均取得了不错的疗效,但对于最有效的治疗方式仍有争议,因此本文通过检索国内外相关文献进行分析研究,对二分髌骨的研究现状进行综述,内容包括二分髌骨的流行病学、发病机制、临床表现、鉴别诊断、治疗等方面,为临床诊断和治疗提供参考。
Bipartite patella(BP)is a relatively rare knee joint disease in clinical practice.As it is usually asymptomatic,it is often overlooked by patients and orthopaedic surgeons.With the current research on BP,the pathogenesis of patella is mainly caused by the non-fusion of ossification center during development,which results in the formation of two bone blocks without osseous association.The present diagnostic methods include knee joint X-ray,CT and MRI,in which,MRI is regarded as the gold standard in the diagnosis of BP.The treatment methods include conservative treatment,parapellopatectomy,soft tissue lysis and reduction and internal fixation of parapellopatella,and all the therapies have obtained satisfactory curative effects,however,the most effective therapy is still controversial,therefor,this article retrieves related literature at home and abroad research to summarized the research current situation of BP including binary patellar epidemiology,pathogenesis,clinical manifestations,differential diagnosis and treatment,so as to provide reference for clinical diagnosis and treatment.
