摘要
目的探讨外周血雌激素受体β(ERβ)基因AluI位点多态性与妊娠期糖尿病(GDM)易感性的关系。方法选择GDM孕妇500例(观察组)和糖耐量正常孕妇500例(对照组),采集两组空腹肘静脉血,采用TaqMan探针SNaPshot基因分型技术检测外周血ERβ基因AluI位点多态性,比较不同基因型者空腹血糖(FPG)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)、TC、TG。收集所有研究对象年龄,孕前BMI、收缩压、舒张压和糖尿病家族史,妊娠24~28周血生化指标(FPG、FINS、HOMA-IR、TC、TG),妊娠早期和妊娠中期体质量增加以及外周血ERβ基因AluI位点多态性等临床相关资料,分析影响GDM发生的独立危险因素。结果两组外周血ERβ基因AluI位点基因型和等位基因分布比较差异均有统计学意义(χ2分别为35.425、36.153,P均<0.05)。携带ERβ基因AluI位点AA、Aa基因型孕妇GDM的发生风险明显高于携带aa基因型孕妇(OR分别为1.726、1.439,95%CI分别为1.235~8.263、1.056~4.267),携带ERβ基因AluI位点A等位基因孕妇GDM的发生风险明显高于携带a等位基因孕妇(OR=1.821,95%CI:1.403~12.051)。GDM患者外周血ERβ基因AluI位点AA、Aa基因型者FPG、FINS、HOMA-IR、TC均高于aa基因型者(P均<0.05)。单因素分析发现,观察组孕前BMI、孕前收缩压、糖尿病家族史、FPG、FINS、HOMA-IR、TC、妊娠早期体质量增加、妊娠中期体质量增加均高于对照组(P均<0.05),而两组年龄、孕前舒张压、TG比较差异均无统计学意义(P均>0.05)。二元Logistic回归分析发现,FPG、糖尿病家族史、ERβ基因AluI位点多态性是影响GDM发生的独立危险因素(P均<0.05)。结论外周血ERβ基因AluI位点多态性与GDM易感性密切相关,携带A等位基因孕妇可能是GDM易感的高危人群,并且与GDM糖脂代谢紊乱和胰岛素抵抗有一定关系。
Objective To investigate the association between AluI locus polymorphism of estrogen receptorβ(ERβ)gene and the susceptibility to gestational diabetes mellitus(GDM).Methods Totally 500 pregnant women with GDM(GDM group)and 500 pregnant women with normal glucose tolerance(control group)were selected,and the fasting elbow venous blood were collected.The AluI locus polymorphism of ERβgene in the peripheral blood was detected by Taq⁃Man probe SNaPshot genotyping technique.Fasting blood glucose(FPG),fasting insulin(FINS),insulin resistance in⁃dex(HOMA-IR),TC,and TG in pregnant women with different genotypes were compared.The age,progestational BMI,systolic pressure,diastolic pressure,and DM family history,blood biochemical indexes(FPG,FINS,HOMA-IR,TC,TG)at 24-28 weeks of gestation,the increased body weight during the early trimester and midtrimester of pregnancy,and the AluI locus polymorphism of ERβgene in the peripheral blood and other clinical data were collected.The independent risk factors for GDM were analyzed.Results There were significant differences in the ERβgene AluI locus genotypes and allele distribution in the peripheral blood(χ2=35.425,36.153;all P<0.05).The risks of GDM in pregnant women with ERβgene AluI locus AA genotype and Aa genotype were significantly higher than that of pregnant women with aa gen⁃otype(OR=1.726,1.439;95%CI:1.235-8.263,1.056-4.267,respectively).The risk of GDM in pregnant women with ERβgene AluI locus A allele was significantly higher than that of pregnant women with a allele(OR=1.821,95%CI:1.403-12.051).The FPG,FINS,HOMA-IR,and TC in peripheral blood of GDM pregnant women with ERβgene AluI locus AA genotype and Aa genotype were higher than those of GDM pregnant women with aa genotype(all P<0.05).Univariate analysis showed that the pregestational BMI,pregestational systolic pressure,DM family history,FPG,FINS,HOMA-IR,TC,early pregnancy weight increase,and mid pregnancy weight increase were higher in the observation group than in the control group(all P<0.05),while there were no significant differences in age,diastolic pressure before preg⁃nancy or TG between the two groups(all P<0.05).Binary Logistic regression model showed that FPG,DM family history and AluI locus polymorphism of ERβgene were independent risk factors for GDM(all P<0.05).Conclusion The AluI locus polymorphism of ERβgene in the peripheral blood is closely related to GDM susceptibility,and allele A carriers may be the high-risk population of GDM susceptibility,which may be associated with glucose and lipid metabolism disorder and insulin resistance of GDM.
