脑电图联合血氨检测对肝性脑病的诊断价值 被引量:3

摘要 目的分析脑电图联合血氨检测对肝性脑病的诊断价值。方法随机选取2014年5月至2019年8月北京市海淀医院收治的112例肝硬化患者。根据其是否患有肝性脑病(HE)分为无肝性脑病(n HE)组62例,伴有HE组50例。比较分析两组患者脑电图异常表现情况和血氨指标水平。结果伴有HE组患者的脑电图异常率为82.00%,明显高于n HE组的12.90%,差异有统计学意义(P<0.05)。伴有HE组患者血氨浓度值为(117.82±46.24)μg/dl,明显高于n HE组患者的(40.57±21.73)μg/dl,差异有统计学意义(P<0.05)。脑电图联合血氨检测肝性脑病的敏感度、准确率明显高于脑电图、血氨单一检测。结论脑电图联合血氨检测可显著提高肝性脑病患者诊断敏感度、准确率,具有较高的临床应用价值。 Objective To analyze the diagnostic value of EEG combined with blood ammonia detection in hepatic encephalopathy.Methods A total of 112 patients with liver cirrhosis admitted to Beijing Haidian Hospital from May 2014 to August 2019 were randomly selected.According to whether they had hepatic encephalopathy(HE),they were divided into the nonhepatic encephalopathy(nHE)group(n=62)and the HE group(n=50).The abnormal performance of EEG and the level of blood ammonia were compared and analyzed between the two groups of patients.Results The abnormal rate of EEG in patients of the HE group was 82.00%,significantly higher than 12.90%of patients in the nHE group,with statistically significant difference(P<0.05).The blood ammonia concentration in patients of the HE group was(117.82±46.24)μg/dl,significantly higher than(40.57±21.73)μg/dl of patients in the nHE group,with statistically significant difference(P<0.05).The sensitivity and accuracy of EEG combined with blood ammonia in detecting hepatic encephalopathy were significantly higher than those of EEG and blood ammonia alone.Conclusion EEG combined with blood ammonia detection can significantly improve the diagnostic sensitivity and accuracy of patients with hepatic encephalopathy,which has relatively high clinical application value.
