摘要
瘙痒是特应性皮炎(AD)最为突出的临床症状,呈现出显著的昼夜节律性,以夜间瘙痒加重为主,治疗夜间瘙痒是控制AD的重要基础。生物钟是指在生理或行为上所表现的以约24 h为周期的生物节律,目前许多研究表明生物钟与AD夜间瘙痒加重密切相关。这些研究提示生物钟基因可通过调控ST2受体表达,诱导白细胞介素-33活化肥大细胞,在皮肤屏障功能、激素分泌水平、肥大细胞活化、炎症介质释放等方面均起到了重要的调节作用,而不仅仅是一个简单的时间概念。本综述旨在了解与时辰相关的过敏生物学,为治疗临床上难治的AD夜间瘙痒提供新的思路。
Pruritus is the most prominent clinical symptom of atopic dermatitis(AD),presenting a significant circadian rhythm,with the aggravation of nocturnal pruritus.The treatment of nocturnal pruritus is an important basis for the control of AD.Biological clock is a biological rhythm with a cycle of about 24 h in physiology or behavior.At present,many studies have shown that the clock is closely related to the aggravation of nocturnal pruritus in AD.These studies suggest that clock gene can induce interleukin-33 to activate mast cells by regulating the expression of ST2 receptor,which plays an important role in regulating skin barrier function,hormone secretion level,mast cell activation and inflammatory mediators release,rather than a simple concept of time.This review aims to understand the biology of allergy related to time,and provide new ideas for the treatment of clinically refractory AD at nocturnal pruritus.
