摘要
目的:对妊娠期妇女进行甲状腺功能性疾病检测,能及时发现甲状腺功能异常(特别是亚临床甲状腺功能减退简称亚临床甲减),采取有效干预措施,维持孕妇血清甲状腺功能在相应孕周正常水平,满足妊娠生理需要,改善妊娠结局。方法:甲状腺功能筛查是抽血化验促甲状腺激素(TSH)及游离四碘甲状腺素(FT4)。某计划生育服务中心选取了2018年上半年门诊475例建册(孕产妇保健手册)孕妇,其中421例行血清TSH值和FT4进行分析。结果:血清TSH低于0.1 mIU//L有10例其中1例伴FT4成倍升高,TSH超过2.5 mIU/L有 82例(其中超过3.0 mIU/L 54例,超过4.0 mIU/L 18例)且FT4在正常范围,干预治疗TSH在正常范围有5例;54例未检血TSH者中有7例孕前甲减干预治疗中,1例孕早期甲亢治疗中。结论:妊娠期亚临床甲状腺功能减退疾病发病率高,多项研究表明其已居于高危妊娠高危因素前五位,建议妊娠期(最好在妊娠8周前)全员进行甲状腺疾病筛查,血清TSH检测纳入婚检和孕检必查项目。
Objective:To study the thyroid functional disease in women during pregnancy,can timely found abnormal thyroid function (especially subclinical hypothyroidism JiaJian),take effective intervention measures to maintain pregnant women serum level of thyroid function in the corresponding conceived telomerase,satisfy the physiological need,pregnancy to improve pregnancy outcome.Methods:the thyroid function screening is exsanguinate assay for thyroid-stimulating hormone (TSH) and free thyroxine (FT4) four iodine.A family planning service center 475 built in the first half of 2018 outpatient volume (maternal health handbook) pregnant women,of whom 421 lines of serum TSH values and FT4 were analyzed.Results:serum TSH below 0.1 mIU /L in 10 cases (one with higher FT4 coupled,TSH mIU / 2.5 L have more than 82 cases (more than 3.0 mIU/L 54 cases,more than 4.0 mIU/L 18 cases) and FT4 in the normal range,intervention treatment of TSH in the normal range in 5 cases;54 not check blood TSH in 7 cases of pregnancy JiaJian intervention treatment,1 case of pregnant early hyperthyroidism therapy.Conclusion:subclinical thyroid function is low incidence of disease is high during pregnancy,a number of studies have shown that it has been one of the high-risk pregnancy risk factor’s top five,suggest that pregnancy (best in 8 weeks after pregnancy) fully screening for thyroid disease,serum TSH testing into check-ups and pregnancy test will check the project.
