摘要
目的观察糖尿病乳酸酸中毒者行血液透析滤过治疗的效果。方法糖尿病乳酸酸中毒者为研究纳入对象,在2019年11月—2020年11月行不同措施治疗,56例样本中,血液透析滤过者28例,普通酸中毒治疗者28例,属实验组、基础组,观察结果。结果治疗前,糖尿病乳酸酸中毒者乳酸、酸碱值无差异,差异无统计学意义(t=0.082、1.829,P=0.935,0.073)。治疗后24 h、48 h,糖尿病乳酸酸中毒者乳酸、酸碱值在实验组、基础组均改善,乳酸在实验组较低,酸碱值在实验组较高,差异有统计学意义(t=13.018、7.705、8.079、11.300,P<0.001)。糖尿病乳酸酸中毒治疗总有效率在实验组、基础组是96.43%、75.00%,实验组总有效率高,差异有统计学意义(χ^(2)=5.250,P=0.022)。糖尿病乳酸酸中毒者治疗并发症在实验组、基础组是10.71%、39.29%,实验组并发症少,差异有统计学意义(χ^(2)=6.095,P=0.014)。结论血液透析滤过有效性高,可使糖尿病乳酸酸中毒者乳酸水平恢复正常,且机体酸碱值逐渐恢复,并发症少。
Objective To observe the effect of hemodiafiltration treatment in patients with diabetic lactic acidosis.Methods Diabetic lactic acidosis patients were included in the study.Different measures were taken from November 2019 to November 2020.Among 56 samples,28 were hemodiafiltration patients and 28 were treated for common acidosis,which were experimental group,basic group,observe the results.Results Before treatment,there was no significant difference in the values of lactic acid and acid-base in diabetic lactic acidosis patients(t=0.082,1.829,P=0.935,0.073).24 h and 48 h after treatment,the values of lactic acid and acid-base of diabetic lactic acidosis patients were improved in the experimental group and the basal group,the values of lactic acid were lower in the experimental group,and the values of acid-base were higher in the experimental group,the difference was statistically significant(t=13.018,7.705,8.079,11.300,P<0.001).The total effective rate of diabetic lactic acidosis in the experimental group and the basic group was 96.43%and 75.00%,the total effective rate in the experimental group was high,and the difference was statistically significant(χ^(2)=5.250,P=0.022).The complications of diabetic lactic acidosis were 10.71%and 39.29%in the experimental group and the basic group.The complications in the experimental group were less,and the difference was statistically significant(χ^(2)=6.095,P=0.014).Conclusion The high efficiency of hemodiafiltration can make the lactic acid level of diabetic lactic acidosis patients return to normal,and the body's pH value will gradually recover with fewer complications.
