摘要
目的:探究人工流产术后即刻放置不同宫内节育器(IUD)短期避孕效果。方法:选取2017年7月—2019年10月本院妇科人工流产者320例,根据术后置器类型分为吉妮致美IUD组和花式IUD组,每组160例。术后随访1年,对比两组术后避孕效果、置器副反应及并发症。结果:两组术中出血量、术后1h及24h出血量、血性恶露持续时间及恶露持续时间均无差异(P>0.05)。吉妮致美组术后绝经综合征(2.5%)、白带增多(1.3%)和不规则出血(3.1%)发生率均低于花式IUD组(8.8%、6.3%、9.4%)(P<0.05),下腹疼痛(3.8%)、IUD脱落(1.3%)、术后1年内带器妊娠(0.6%)率与花式IUD组(5.6%、3.1%、1.3%)无差异(P>0.05)。结论:人工流产术后即刻放置吉妮致美IUD与花式IUD均有较好避孕效果,吉妮致美IUD副反应及并发症更低。
Objective:To explore the short-term contraceptive effect of different intrauterine device(IUD)inserted after induced abortion immediately.Methods:From July 2017 to October 2019,320 women with induced abortion were selected.According to the type of IUD inserted after abortion,these women were divided into group A(160 women with Gynefix IUD inserted)and group B(160 women with HCu IUD inserted).The contraceptive effect,side effect,and complication of the women after 1 year followed up were compared between the two groups.Results:There were no significant differences in intraoperative blood loss,blood loss in postoperative 1 h and 24 h,duration of lochia rubra,and duration of lochia of the women between the two groups(P>0.05).The rates of menopause syndrome,leucorrhea increase,irregular bleeding of the women in group A were 2.5%,1.3%,and 3.1%,respectively,which were significant lower than those(8.8%,6.3%,and 9.4%,respectively)of the women in group B(P<0.05).The rtes of lower quadrant abdominal pain,IUD discharge,and postoperative pregnancy of the women in group A within 1 year after abortion were 3.8%,1.3%,and 0.6%,respectively,which had no significant different from those(5.6%,3.1%,and 1.3%,respectively)of the women in group B(P>0.05).Conclusion:Ginny IUD or HCu IUD inserted after induced abortion immediately have both good effectiveness,but Ginny IUD has less side effect and complication.
