3.0T磁共振图像融合技术评估三叉神经痛责任血管 被引量:2

摘要 目的探讨3.0T磁共振3D-TOF-MRA联合3D-FIESTA-C序列及图像融合技术对于判断原发性三叉神经痛患者责任血管的诊断价值.方法回顾性分析52例原发性三叉神经痛患者的临床及影像资料,所有患者均于术前行3D-TOF-MRA和3D-FIESTA-C扫描并采用图像融合技术进行图像后处理.对比MRI结果及术中所见,分析3.0T磁共振图像融合技术判断原发性三叉神经痛责任血管的准确性.结果症状侧三叉神经受微血管压迫48例,非症状侧微血管神经压迫7例,两者具有统计学差异.对比MRI结果及术中所见,3.0T磁共振图像融合技术判断原发性三叉神经痛患者责任血管的诊断符合率为92.3%.结论3D-TOF-MRA联合3D-FIESTA-C序列并通过图像融合技术可以提供立体的视角来观察三叉神经与邻近微血管的压迫关系,与手术结果有较高的符合率,可以为临床诊疗提供良好的影像学依据. Objective To explore the diagnostic value of 3.0T MRI 3D-TOF-MRA combined with 3D-FIESTA-C sequence and image fusion technology,in the judgment of offending vessels in patients with primary trigeminal neuralgia.Methods The clinical and imaging data of 52 patients with primary trigeminal neuralgia were retrospectively reviewed.All patients were scanned with 3D-TOF-MRA and 3D-FIESTA-C before operation and processed with image fusion technology.The MRI findings and intraoperative findings were compared,and the accuracy of 3.0T MRI image fusion technology in the judgment of the offending vessels of primary trigeminal neuralgia were analyzed.Results There were 48 cases of microvascular compression on the symptomatic side of trigeminal nerve and 7 cases on the non symptomatic side,which were statistically different.Compared with the imaging findings and surgical results,the accuracy of 3.0T MRI image fusion technology in the judgment of the offending vessels of primary trigeminal neuralgia was 92.3%.Conclusion 3D-TOF-MRA combined with 3D-FIESTA-C sequence and image fusion technology can provide a threedimensional perspective to observe the compression relationship between trigeminal nerve and adjacent microvascular,which has a high coincidence rate with the surgical results,and ean provide a good imaging basis for clinical diagnosis and treatment.
