HSP-70预测急性冠脉综合征血管病变及临床预后的价值

The value of HSP-70 in predicting severity of CAD and prognosis of patients with acute coronary syndrome

摘要 目的探讨热休克蛋白-70(heat shock protein,HSP-70)预测急性冠脉综合征(ACS)冠状动脉血管病变及临床预后的价值.方法前瞻性纳入2017年5月—2019年11月无锡市惠山区人民医院收治的179例ACS患者,收集患者临床资料及检测循环HSP-70水平,院外随访6个月记录主要不良心血管事件(MACEs),根据HSP-70中位数(HSP-70=3.48ng/mL)将患者分为高值组(n=90)及低值组(n=89),分析HSP-70与ACS患者病情、预后的相关性,以及其作为预后评价指标的价值.结果与低值组相比,高值组HSP-70、cTnI、CRP、多支血管病变比例及Genesis积分高,同时HSP-70在STEMI亚组、MACEs亚组及多支病变亚组中显著高表达,且HSP-70与cTnI(r=0.521)、CRP(r=0.467)及Genesis积分(r=0.392)呈显著正相关关系,同时Kaplan-Meier生存分析证实高HSP-70组无MACEs事件生存时间较低HSP-70组显著缩短.结论ACS患者高表达HSP-70预示其病情重、预后不佳. Objective To investigate the value of heat shock protein-70(HSP-70)in predicting coronary artery disease and clinical prognosis of acute coronary syndrome(ACS).Methods 179 ACS patients were collected in this study in our hospital between May 2017 and November 2019,the clinical data were collected and level of HSP-70 were detected,major adverse cardiovascular events(MACEs)were recorded for 6 months after discharge,all of them were divided into high group(n=90)and low group(n=S9)according to median of HSP-70(HSP-70=3.48ng/mL),the value of HSP-70 in predicting severity and prognosis of ACS patients were analyzed.Results Compared with low group,the level of HSP-70,cTnI,CRP,rate of multi-vessel disease and Genesis score were higher in high group,meanwhile HSP-70 was significantly higher in STEMI subgroup,MACES subgroup and multi-vessel disease subgroup than others,what's more,HSP-70 had positively correlated with cTnI(r=0.521),CRP(r=0.467)and Genesis score(r=0.392);Kaplan-Meier survival analysis suggested that survival time of patients in high group was significant shorten than low group.Conclusion The high expression of HSP-70 in ACS indicated that patients had state of ACS and poor prognosis.
