摘要
目的基于数据挖掘分析叶天士治疗心悸的用药特点。方法收集叶天士治疗心悸相关医案及处方共计91首,运用中医传承辅助平台(V2.5)软件建立数据库,分析伴随症状出现频次、药物使用频次、药物四气五味及归经出现频次、药对组合出现频次及药对关联。结果伴随症状排名前4位的依次为不寐、眩晕、汗出、遗精。药物使用频次排名前4位的依次为茯神、人参、炙甘草、白芍、生地黄。药物四气以温、平为主,药物五味以甘、苦为主,药物归经以肾、心、肝、脾为主。药对组合出现频次排名前6位的依次为人参-茯神、炙甘草-白芍、阿胶-生地黄、炙甘草-茯神、白芍-茯神、柏子仁-茯神。进一步挖掘核心用药,核心药对组合为茯神-炙甘草-白芍、阿胶-生地黄。结论叶天士治疗心悸以补虚安神、佐以攻邪,交通心肾、镇怯固摄,顾护脾胃,用药轻灵为总则,为当代中医临床实践提供了参考。
Objective To analyze the medication characteristics of Ye Tianshi in the treatment of palpitation based on data mining.Methods A total of 91 medical records and prescriptions related to Ye Tianshi's treatment of palpitation were collected,and the database was established by using the TCM inheritance auxiliary platform(V2.5)software.The frequency of accompanying symptoms,frequency of drug use,frequency of four qi and five flavors and meridian tropism of drugs,frequency of drug pair combination and drug pair association were analyzed.Results The top four accompanying symptoms were insomnia,vertigo,sweating and spermatorrhea.The top four drug use frequency were fushen,ginseng,fried licorice,radix paeoniae alba and rehmannia glutinosa.The four qi of drugs are mainly warm and flat,the five flavors of drugs are mainly sweet and bitter,and the meridian tropism of drugs are mainly kidney,heart,liver and spleen.The top six drug pair combinations were ginseng-fushen,fried licorice-radix paeoniae alba,donkey hide gelatin-rehmannia glutinosa,fried licorice-fushen,radix paeoniae alba-fushen,platycladus-fushen.Further explore the core drugs,the core drug pairs are fuzhen-fried licorice-radix paeoniae alba and rehmannia glutinosa-donkey hide gelatin.Conclusion The general principles of Ye Tianshi's treatment of palpitation are tonifying deficiency and calming the nerves,assisting to attack pathogenic factors,communicating with heart and kidney,calming fear and taking care of the spleen and stomach,and using drugs with lightness and flexibility,which provide a reference for contemporary clinical practice of traditional Chinese medicine.
