摘要
目的分析高频彩色多普勒超声在血友病膝关节病变检查中的应用价值。方法选取30例血友病患者设置为病例组,另选取同期体检的30例健康者作为对照组。采取高频彩色多普勒超声对两组研究对象双侧膝关节进行检查,对比两组的滑膜厚度、髌上囊积液厚度、膝关节内彩色血流分级等指标,并对比不同严重程度的血友病患者的超声评分。结果30例血友病患者共60个膝关节,检出55个膝关节滑膜增厚,54个膝关节髌上囊积液增厚,41个膝关节超声探及血流信号。病例组与对照组滑膜厚度、髌上囊积液厚度、滑膜彩色血流状况各级比例对比,差异均具有统计学意义(P<0.05)。轻度、中度、重度血友病患者间膝关节超声评分对比,差异均具有统计学意义(P<0.05)。结论高频彩色多普勒超声能有效检出血友病膝关节病变,且能够初步评估不同程度病情,值得临床推广应用。
Objective To analyze the practical value of high frequency color Doppler ultrasound in the examination of hemophilic arthropathy of the knee.Methods There were 30 hemophilia patients selected as case group,and another 30 healthy subjects as the control group.High-frequency color Doppler ultrasound was used to examine the bilateral knee joints of the two groups.Comparison was made on synovial thickness,suprapatellar sac effusion thickness,color blood flow grading in the knee joint between the two groups,and ultrasound scores of hemophilia patients of different severity.Results In 30 hemophilia patients(60 knee joints),55 knee joints were detected with thickened synovium,54 knee joints with thickened suprapatellar sac,and 41 knee joints were detected with ultrasound and blood flow signals.There was statistically significant difference in synovial membrane thickness,suprapatellar sac effusion thickness,and synovial color blood flow grading between the case group and control group(P<0.05).There was statistically significant differences in ultrasound scores of knee jont among mild,moderate and severe hemophilia patients(P<0.05).Conclusion High frequency color Doppler ultrasound can effectively detect hemophilic arthropathy of the knee,and can preliminarily assess the severity of disease,which is worthy of clinical promotion and application.
