脑室引流管外接压力传感器监测颅内压的临床应用 被引量:1

摘要 目的:探讨脑室引流管外接压力传感器监测颅内压的临床应用效果。方法:选择2019年8月-2020年1月本院脑室穿刺引流患者120例为对象,随机数字表法分为两组,各60例。对照组采用有创颅内压监测,观察组采用脑室引流管外接压力传感器监测颅内压,对于颅内压水平稳定在15 mm Hg时完成颅内压监测,监测完毕后对患者效果进行评估并完成6个月随访,比较两组颅内压水平、治疗预后、甘露醇使用情况、并发症发生率。结果:所有患者均完成颅内压监测,监测时间2~15 d,平均(6.93±1.14)d;对照组颅内压监测平均水平(14.39±1.53)mm Hg,与观察组的(14.34±1.51)mm Hg比较,差异无统计学意义(P>0.05);两组重度残疾率、植物生存率差异无统计学意义(P>0.05);观察组治疗后恢复良好率、轻度残疾率均高于对照组(P<0.05);死亡率均低于对照组(P<0.05);观察组甘露醇使用时间、甘露醇使用量短/少于对照组(P<0.05);观察组治疗后肺部感染、颅内感染、电解质紊乱、再次脑出血及应激性溃疡等并发症发生率均低于对照组(P<0.05)。结论:脑室引流管外接压力传感器用于颅内压监测中能动态了解患者颅内压水平,可获得良好治疗预后,缩短/减少甘露醇使用量,有助于降低并发症发生率,值得推广应用。 Objective:To investigate the clinical effect of monitoring intracranial pressure by external pressure sensor connected with ventricular drainage tube.Method:A total of 120 patients with ventricular puncture and drainage in our hospital from August 2019 to January 2020 were selected as subjects,and randomly divided into two groups of 60 each by number table method.Control group using invasive intracranial pressure monitoring,observation group using pressure sensor monitoring of intracranial pressure,pipe external ventricular drainage for intracranial pressure stable level completed in 15 mm Hg intracranial pressure monitoring,monitoring assessment and complete the effect of the patients after 6 months follow-up,compared two groups of intracranial pressure levels,complications,treatment,prognosis,mannitol usage.Result:All patients completed the monitoring of intracranial pressure (ICP) for 2-15 days,with an average of (6.93±1.14) days.The average intracranial pressure (ICP) of the control group was (14.39±1.53) mm Hg,and the ICP of the observation group was (14.34±1.51) mm Hg,there was no statistical significance (P>0.05).There was no significant difference in severe disability rate and plant survival rate between the two groups (P>0.05).The good recovery rate and mild disability rate of observation group were higher than those of control group (P<0.05).The mortality was lower than that of the control group (P<0.05).The duration of mannitol use and the amount of mannitol in observation group were shorter/less than those in control group (P<0.05).After treatment,the incidence of pulmonary infection,intracranial infection,electrolyte disturbance,recurrent cerebral hemorrhage and stress ulcer in the observation group were lower than those in the control group (P<0.05).Conclusion:The external pressure sensor connected with ventricular drainage tube can dynamically understand the intracranial pressure level of patients,obtain good treatment prognosis,shorten/reduce the use of mannitol,help to reduce the incidence of complications,and is worthy of popularization and application.
