妊娠期糖尿病合并胰岛素抵抗产妇最佳引产时间分析 被引量:1

Analysis of Optimal Time of Labor Induction in Pregnant Women with Gestational Diabetes Mellitus and Insulin Resistance

摘要 目的:探究妊娠期糖尿病合并胰岛素抵抗产妇的最佳引产时间。方法:回顾性分析2019年3月-2020年3月在笔者所在医院实施引产的糖尿病合并胰岛素抵抗产妇103例,按照不同引产时机分为三组,A组30例(孕周37~38周),B组38例(孕周39~40周),C组35例(孕周40周以上)。对比各组血糖水平、引产方式和妊娠结局。结果:三组孕妇空腹血糖(FBG)、空腹胰岛素(FNS)、餐后2 h血糖及胰岛素、糖化血红蛋白水平差异无统计学意义(P>0.05);三组患者产道裂伤、剖宫产率比较差异无统计学意义(P>0.05),但B组产后出血率、胎膜早破率显著低于A组、C组(P<0.05);三组患者新生儿中巨大儿、低血糖、呼吸窘迫发生率差异无统计学意义(P>0.05),但B组新生儿窒息率明显低于A、C组,且B组新生儿死亡率明显低于A组(P<0.05)。结论:妊娠期糖尿病合并胰岛素抵抗产妇的最佳引产时间为39~40周,明显改善了产后母婴结局,引产效果显著。 Objective:To investigate the optimal induction time of pregnant women with gestational diabetes mellitus and insulin resistance.Method:It was analyzed retrospectively 103 women with diabetes and insulin resistance who underwent induced labor in our hospital from March 2019 to March 2020,which according to different induction times,these women were divided into three groups,including 30 patients in group A (37-38 weeks gestation),38 patients in group B (39-40 weeks gestation),and 35 patients in group C (over 40 weeks gestation),then compared the blood glucose levels,delivery patterns and pregnancy outcomes among groups.Result:There were no statistically significant differences in fasting blood glucose (FBG),fasting insulin (FNS),2 h postprandial blood glucose,insulin and glycated hemoglobin levels among the three groups (P>0.05),the differences in those results of the birth canal laceration rate,the cesarean section rate and the incidence of macrosomia,hypoglycemia as well as respiratory distress among three groups were also insignificant (P>0.05).However,the rate of postpartum hemorrhage,premature rupture of membranes and neonatal asphyxia in group B were signally lower than those in group A and C,and neonatal mortality in group B was observably lower than that in group A (P<0.05).Conclusion:The optimal time of labor induction in pregnant women with gestational diabetes mellitus and insulin resistance is 39-40 weeks,which obviously improves the outcomes of postpartum mothers and the labor induction effect is significant.
