摘要
目的分析综合医院外科超长期住院患者的分布情况及其影响因素,为有效预防、控制不合理超长期住院,提高医疗资源利用率提供参考依据。方法对某院2019年外科系统870例超长期住院患者的性别、年龄、出院科室等分布特征进行统计描述,采用单因素分析及logistic回归分析探索超长期住院形成的可能影响因素。结果某院外科超长期住院患者平均住院日41.54(32.77,44.76)天,平均年龄为(51.81±17.37)岁,超长期住院发生率前三位科室为神经外科(3.46%)、普外科(1.23%)、胰腺外科(0.86%)。logistic回归分析显示,急诊入院、转科、三四级手术、二次及以上手术是外科住院患者发生超长期住院的影响因素。结论医务管理部门应对重点科室的转科、急诊入院、二次手术等节点做重点把控,采取多种措施控制、干预不合理超长期住院的发生,有效提高医疗服务效率。
Objective To analyze the distribution characteristics and influencing factors of surgical pa⁃tients hospitalized over 30 days in a general hospital and to provide reference for effectively controlling irrational ex⁃tra-long hospitalization days and improving the use of medical resources.Methods The distribution characteristics of hospitalization time,gender,age,and discharge department for the 870 patients in the surgical departments of a hospital were analyzed.The univariate analysis and logistic regression analysis were used to explore the possible in⁃fluencing factors for the existence of extra-long hospitalization days.Results The average length of hospitalization for surgical long-term inpatients in the hospital was 41.54(32.77,44.76)days,and the average age was(51.81±17.37).The top three departments with incidence of extra-long hospitalization were neurosurgery(3.46%),gener⁃al surgery(1.23%),and pancreatic surgery(0.86%).Logistic regression analysis showed that the admission route,number of operations,operation level,and transferal were the possible influencing factors for the extra-long hospitalization of surgical patients.Conclusion The medical management department should focus on the transfer of departments and secondary operations in key departments and take a variety of measures to control the occurrence of unreasonable and extra-long hospitalization,so as to effectively improve the efficiency of medical services.
