摘要
目的:观察女性盆腔双侧附件病变的一般临床表现、血清学指标、超声、CT/MRI表现,探讨CT/MRI的诊断价值。方法:回顾性分析237例经病理及临床治疗证实的盆腔双侧附件均有病变的患者,根据病变成分分为3组,观察各组的年龄分布、临床首发症状、血清肿瘤标志物、双侧病理一致性、并发盆腔炎症情况,以及超声与CT/MRI诊断的差异性。结果:良性病变占76.8%(182/237),其中80.2%(146/182)以囊性成分为主(P<0.001)。在双侧附件病变的一致性上,良性病变中子宫内膜异位性囊肿双侧发生的概率最高,恶性病变以囊腺癌双侧发生的概率最高(均P<0.001)。CT/MRI对双侧附件病变的成分分析及最终诊断正确率均较超声检查高(P<0.05,P<0.01),且对实性病变的诊断优势更明显(P<0.001)。结论:CT/MRI对女性盆腔双侧附件病变的诊断正确率较高,尤其对实性成分偏多病变较超声具有更明显的诊断优势。
Objective:To investigate the clinical manifestations,serological indexes,ultrasound and imaging findings of bilateral adnexal lesions in female pelvis and explore the diagnostic value of CT and MRI.Methods:A total of 237 patients with bilateral adnexal lesions confirmed by pathology and clinical treatment were retrospectively analyzed.The patients were subsequently divided into three groups according to the tissue components of lesions.The age distribution,initial clinical symptoms,serum tumor markers,consistency of bilateral pathology,complicated with pelvic inflammation and the difference of ultrasound and CT/MRI diagnosis were observed.Results:One hundred and eighty-two patients(76.8%)had benign bilateral adnexal lesions,including 146(80.2%)predominantly cystic lesions(P<0.001).As far as the consistency of bilateral lesions was concerned,the incidence of bilateral endometriotic cysts was the highest in benign lesions and the incidence of bilateral cystadenocarcinoma was the highest in malignant lesions(both P<0.001).The accuracy of CT/MRI in component analysis and final diagnosis of bilateral adnexal lesions was higher than that of ultrasound(both P<0.05)and the advantage was more obvious in solid lesions(P<0.001).Conclusion:CT/MRI is superior to ultrasound in the diagnosis of bilateral pelvic adnexal lesions,especially for predominantly solid lesions.
