摘要
糖尿病神经源性膀胱(DNB)病因病机较为复杂,目前尚无确切有效的根治方法,严重影响了糖尿病患者的生命健康及生活质量。中医将本病归为"消渴""癃闭"范畴,认为该病病位在肾与膀胱,多因二者气化不利所致。人体是一个有机的整体,祖国医学是以中医基础理论为指导,整体观念为中心,辨证论治为基础,因此在治疗DNB时需重视各个脏腑的作用及他们之间的病理变化,不可仅拘泥于肾与膀胱,故从古代临床著作与现代医家治疗DNB治疗经验出发,从多脏腑角度以探析、总结治疗DNB理论依据及治疗方法。另一方面,络病学说是中医理论体系重要组成部分,基于叶氏"久病入络""络以通为用"观点对DNB病因病机及治疗原则从络病理论进行分析,是探讨DNB又一辨治思路。
The etiology and pathogenesis of diabetic neurogenic bladder(DNB)are complex,and there is not exact and effective radical cure method at present,which seriously affects the healthy life and quality of life of diabetic patients. Traditional Chinese medicine(TCM)classified the disease as"Xiaoke""uroschesis"and considered that the location of the disease was in the kidney and bladder,and the disease was caused by the disadvantageous gasification of them. The human body is an organic whole. Traditional Chinese medicine is based on the basic theory of traditional Chinese medicine,the overall concept as the center,and syndrome differentiation and treatment as the basis. Therefore,in the treatment of DNB,we should pay attention to the role of each viscera and the pathological changes among them,not just confined to the kidney and bladder. Therefore,based on the ancient clinical works and modern doctors’ experience in treating DNB,this paper analyzes and summarizes the theoretical basis and treatment of DNB from the perspective of multiple viscera. On the other hand,collateral disease theory is an important part of the theoretical system of traditional Chinese medicine. Based on YE’s view points of"chronic diseases entering collaterals"and"collaterals are used for unblocking",the etiology,pathogenesis and treatment principles of DNB are analyzed from the theory of collateral diseases,so as to explore another thinking of differentiation and treatment of DNB.
