摘要
目的观察督脉活血汤联合理筋手法对退行性腰椎管狭窄症(DLSS)患者腰椎功能及炎性因子的影响。方法将我院104例DLSS患者随机分为观察组与对照组各52例。对照组患者口服塞来昔布和甲钴胺进行治疗,观察组在对照组口服用药的基础上采用内服督脉活血汤、外用理筋手法进行治疗,比较两组临床疗效、治疗前后两组的腰椎功能、腰痛程度、日常生活能力及血清中炎性因子的表达情况。结果观察组总有效率高于对照组(P<0.05);治疗后,两组日本骨科协会评估治疗分数(JOA)评分增加,视觉模拟评分法(VAS)、中文版Oswestry disability index功能障碍指数量表(ODI)评分均降低且观察组JOA评分高于对照组,VAS、ODI评分均低于对照组(P<0.05);两组肿瘤坏死因子-α(TNF-α)、白介素-1β(IL-1β)、单核细胞趋化蛋白1(MCP-1)、降钙素原(PCT)水平均降低且观察组水平均低于对照组(P<0.05)。结论督脉活血汤联合理筋手法治疗DLSS疗效较好,能显著地改善腰椎功能,降低腰痛程度提高日常生活能力,降低血清中炎性因子的表达水平。
Objective To observe the effects of Dumai Huoxue Decoction combined with reasonable tendon manipulation on lumbar spine function and inflammatory factors in patients with degenerative lumbar spinalstenosis(DLSS).Methods 104 patients with DLSS were randomly divided into observation group and control group,52 cases each.Control group was treated with celecoxib and mecobalamin orally,and observation group was treated with Dumai Huoxue Decoction and external tendon therapy on the basis of control group.The clinical efficacy,the function of lumbar spine,the degree of back pain,the ability of daily living and the expression of inflammatory factors in the serum of the two group were compared before and after treatment.Results The total effective rate of observation group was higher than control group(P<0.05);after treatmeng,the Japanese Orthopaedic Association Assessment(JOA) treatment score of both groups enhanced,and the Visual analogue scale(VAS)score and Chinese version of Oswestry disability index(ODI) decreased;the JOA score of observation group was higher than control group,VAS and ODI scores of observation group were lower than control group(P<0.05);the levels of tumor necrosis factor-α(TNF-α),interleukin-1 β(IL-1β),monocyte chemoattractant protein 1(MCP-1) and procalcitonin(PCT) in both groups decreased after treatment,which in observation group were lower than those in control group(P<0.05).Conclusion Dumai Huoxue Decoction combined with reasonable tendon manipulation is satisfactory for the efficacy of DLSS,which significantly improves lumbar spine function reduces the degree of low back pain,improves the ability of daily living,and reduces the expression level of serum inflammatory factors.
