红外热成像评估定点侧屈旋扳手法治疗神经根型颈椎病的可视化研究 被引量:10

A Visual Study of Fixed-Point Lateral Flexion-Rotational Manipulation on Cervical Spondylotic Radiculopathy by Infrared Thermal Imaging

摘要 目的应用红外热成像可视化热图评估定点侧屈旋扳手法治疗神经根型颈椎病的临床疗效及安全性。方法选取120例神经根型颈椎病患者按随机数字表法分为治疗组(定点侧屈旋扳手法)与对照组(非定点仰卧三牵手法),每组各60例,隔日1次,15~20 min/次,14次为1个疗程,治疗1个疗程。比较两组治疗前后颈臂疼痛视觉模拟评分(VAS)、颈椎曲度、患侧红外热成像温度均值及临床疗效。结果治疗结束后,两组的VAS评分及红外热成像温度均值均较治疗前降低(P<0.05),颈椎曲度均较治疗前改善(P<0.05);治疗组VAS评分及红外热成像温度均值降低优于对照组(P<0.05),颈椎曲度改善优于对照组(P<0.05);治疗组总有效率高于对照组(P<0.05)。两组均无不良反应发生。结论定点侧屈旋扳手法治疗神经根型颈椎病疗效优于非定点仰卧三牵手法,疗效确切,安全性高,红外热成像可视化热图可作为手法治疗神经根型颈椎病临床疗效的一种评价指标。 Objective To evaluate the clinical effect and safety of the fixed-point lateral flexion-rotational manipulation for cervical spondylotic radiculopathy by the visible heat map of infrared thermal imaging.Methods A total of 120 patients were divided into treatment group and control group,60 cases in a group.The fixed-point lateral flexion-rotational manipulation was used in the treatment group.The non-fixed point supine and three pull manipulation was used in the control group,once every other day,15-20 min/time,14 times for a course of treatment.The visual analogue scale(VAS)score and cervical curvature was compared in two groups.The VAS score,cervical curvature,mean value of infrared imaging temperature and clinical efficacy were compared between the two groups before and after treatment.Results After treatment,the VAS scores and the mean value of infrared imaging temperature were significantly decreased(P<0.05).The curvature of the cervical spine was improved(P<0.05).VAS score and mean reduction of infrared imaging temperature in the treatment group were better than those in the control group(P<0.05),and the cervical curvature improvement was better than that in the control group(P<0.05).The total effective rate of the treatment group was higher than that of the control group(P<0.05).No adverse reactions in either group.Conclusion The curative effect of the fixed-point lateral rotation manipulation in the treatment of cervical spondylotic radiculopathy is better than that of the non-fixed-point supine and three pull manipulation.The efficacy is definite,the safety is high and the visible heat map of infrared thermal imaging can be used as an evaluation index for the clinical efficacy of manipulation in the treatment of cervical spondylotic radiculopathy.
