sFlt-1和AQP1表达与糖尿病肾病的相关性 被引量:1

Correlation between sFlt-1 and AQP1 expression and diabetic nephropathy

摘要 目的探讨可溶性血管内皮生长因子受体1(sFlt-1)和水通道蛋白1(AQP1)表达与糖尿病肾病(DN)的相关性研究。方法选取2015年至2018年江苏省南通大学附属海安医院和南通大学附属医院收治的DN患者120例,根据患者尿白蛋白排泄率(UAER)分为三组:<30 mg/24 h的单纯糖尿病组(SDM组).30~300 mg/24 h的隐性糖尿病组(IDN组).>300 mg/24 h的显性糖尿病肾病组(ODN组),各40例。选取同期健康体检者40例为对照组。检测患者血清sF1t-1和AQPI变化。结果DN患者随UAER值升高,AQP1水平显著升高(P<0.05),而sFl-1水平显著下降(P<0.05),联合检测DN患者血清内sFl-1和AQP1的灵敏度、特异性、阳性预测值均高于各指标单项检测(P<0.05),阴性预测值显著低于各指标单项检测(P<0.05);DN的严重程度与血液AQP1存在显著的正相关,与sF1t-1存在显著的负性相关(P<0.05),sFl-1和AQPI是DN发病的独立危险因素(P<0.05)。结论DN患者血清中sFlt-l和AQPI含量,对患者病情的提示与诊断具有重要的作用。 Objective To investigate the relationship between the expression of sFlt-1 and AQP1 and diabetic nephropathy(DN).Methods From 2015 to 2018,120 patients with DN admitted to Haian hospital afiliated to Nantong university and Nantong university afiliated to Jiangsu province were enrolled.According to the urinary albumin excretion rate(UAER),the patients were divided in-to three groups:UAER<30 mg/24 h(SDM group),UAER 30-300 mg/24 h,(IDN group),UAER>300 mg/24 h(ODN group),40 cases in each group.Forty healthy people were selected as the control group during the same period.Changes in serum sFlt-I and AQP1 were measured in pa-tients.Results AQP1 levels were significantly increased with UAER values in patients with DN,and sFlt-1 levels were significantly decreased(P<0.05).Sensitivity,specificity,positive predictive value and negative predictive value of serum sFlt-1 and AQP1 in patients with DN were higher than the combined detection of each index(P<0.05).There was a significant positive linear correlation be-tween the severity of DN in the DM,IDN and ODN groups and the blood AQP1 of the patients,and a significant negative correlation with sFlt-1(P<0.05),sFlt-1 and AQP1 was an independent risk factor for the onset of DN(P<0.05).Conclusions The serum levels of sFlt-1 and AQP1 in patients with DN play an important role in the prompting and diagnosis of patients'condition.
