摘要
目的评价麻腮风联合减毒活疫苗(measles,mumps and rubella combined vaccine,live,MMR)的免疫原性,同时比较采用其和腮腺炎减毒活疫苗(mumps vaccine,MV)加强免疫后的抗体水平,为制定新的免疫策略提供依据。方法选择2017-2018年入伍的176名新兵、上等兵为观察对象,随机分为两组,其中第一组第一针接种MMR,30 d后再次接种MMR;第二组接种MV。对两组受试者进行随访、采血,实验室检测腮腺炎抗体水平,同时采用ELISA方法与微量血凝抑制试验(hemagglutination inhibition,HI)进行对比,统计分析其抗体水平的变化。结果两组之间抗体阳性率、阳转率、几何平均滴度(GMT)、几何平均滴度增长倍数(GMI)、抗体水平分布差异无统计学意义(P>0.05)。第一轮两组受试者都接种MMR,FAS数据集和PPS数据集腮腺炎病毒抗体阳性率分别为97.73%、97.70%,阳转率分别为35.68%、36.21%,GMT分别为14.44、14.66,GMI分别为2.57、2.60。强化接种后两组之间抗体阳性率分别为100.00%、98.81%;阳转率分别为63.64%、54.55%;GMT分别为28.43、28.11;GMI分别为4.23、4.29。两组之间抗体阳性率、阳转率、GMT、GMI、抗体水平分布等指标差异均无统计学意义(P>0.05)。结论MMR和MV对受试者的加强免疫效果差异无统计学意义,加强免疫接种MV或MMR均可。
Objective To evaluate the immunogenicity of measles,mumps and rubella combined vaccine,live(MMR).To compare the antibody levels after booster immunization with MMR and mumps vaccine(MV),and to provide a reference for formulating new immunization strategies.Methods The 176 recruits and superior soldiers enlisted in the army from 2017 to 2018 were selected as observation objects and randomly divided into two groups,of which the first group was inoculated with MMR by the first needle and then inoculated with MMR again after 30 d;the second group was inoculated with MV.The subjects in the two groups were followed up,blood was collected on time,and the antibody level of mumps was measured in the laboratory.Meanwhile,the changes in their antibody levels were statistically analyzed using the ELISA method compared to the micro-hemagglutination inhibition(HI)method.Results There were no statistically significant differences in antibody positivity rate,positivity conversion rate,geometric mean titer(GMT),geometric mean titer growth multiplier(GMI),and antibody level distribution between the two groups(P>0.05).Both groups of subjects were vaccinated with MMR in the first round.The positivity rates of mumps virus antibody were 97.73%,97.70%in the FAS data set and PPS data set;the positivity conversion rates of mumps virus antibody were 35.68%,36.21%;the GMT were 14.44,14.66;the GMI were 2.57,2.60.After the booster vaccination,the positivity rates of antibody against mumps were 100.00%,98.81%;the positivity conversion rates of antibody against mumps were 63.64%,54.55%;the GMT were 28.43,28.11;the GMI were 4.23,4.29.There were no statistically significant differences in antibody positivity rates,positivity conversion rates,GMT,GMI and antibody level distribution between the two groups(P>0.05).Conclusion There was no statistically significant difference in booster immunization effects between MMR and MV,booster immunization with either MV or MMR is acceptable.
