摘要
目的探讨超声联合弹性成像在非哺乳期乳腺炎与乳腺癌鉴别诊断中的临床价值。方法选取来我院就诊的临床诊断为非哺乳期乳腺炎的患者54例作为观察组,同时选取来我院就诊的临床诊断为乳腺癌的患者48例作为对照组,两组均进行常规超声检查及弹性成像检查,且均进行病理组织学检查,然后分析两组患者的常规超声及弹性成像的图像特征、病理组织学检查结果,并对常规超声联合弹性成像技术对两种疾病的诊断价值进行分析。结果非哺乳期乳腺炎患者病灶中微小钙化的检出率(7.4%),后方回声衰减的检出率(20.4%),血流等级为Ⅱ~Ⅲ级的比例(59.3%),弹性评分4~5分者占比(13.0%)以及RI值≥0.70患者的比例(48.1%)均低于乳腺癌组,差异具有统计学意义(P<0.05);超声联合弹性成像技术对非哺乳期乳腺炎和乳腺癌患者诊断的准确率均高于常规超声。结论采用超声联合弹性成像对非哺乳期乳腺炎和乳腺癌进行鉴别诊断时,能够清晰显示两种疾病的不同影像学特征,且诊断准确率较高。
Objective To analyze the clinical differential diagnostic value of ultrasound combined with ultrasounic elastography for non-lactating mastitis and breast cancer.Methods 54 patients who were diagnosed with non-lactating mastitis from our hospital were collected as observation group,48 patients who were diagnosed with breast cancer were collected as control group at the same time,routine ultrasonography,elastography and histopathological examination were performed in both groups,then the image features and histopathological findings between the conventional ultrasound and elastography were analyzed,and the diagnostic value of conventional ultrasound combined with elastography in two diseases was also analyzed.Results The detection rate of microcalcification(7.4%),the detection rate of posterior echo attenuation(20.4%),the proportion of blood flow grade II to III in tumors(59.3%),the proportion of elastic score 4~5(13.0%),the proportion of patients with RI≥0.70(48.1%)in non-lactating breast cancer patients were lower than those of breast cancer group(P<0.05).The diagnostic accuracy of ultrasound combined with elastography in non-lactating mastitis and breast cancer patients was both higher than the conventional ultrasound.Conclusion Ultrasound combined with elastography in differential diagnosis of non-lactating mastitis and breast cancer can clearly show the different imaging characteristics of the two diseases,and the diagnostic accuracy is high.
