摘要
目的探讨参数优化后的三维快速自旋回波序列(3D sampling perfection with application optimized contrasts using different flip angle evolutions,3D-SPACE)在儿童磁共振胰胆管造影(magnetic resonance cholangiopancreatogaphy,MRCP)检查中提升图像质量的应用价值。方法对30例患儿采用经优化参数3D-SPACE序列及常规参数3D-SPACE序列进行MRCP扫描,对两种扫描序列得到的图像进行对比分析,比较两种扫描的质量差异。结果经优化后的3D-SPACE序列扫描时间是(50±7)s,常规3D-SPACE序列扫描时间是(210±33)s,2种序列间差异有统计学意义(t=15.95,P=0.000)。优化参数3D-SPACE序列得4分图像的24例,占80%(24/30);3分图像3例,占10%(6/30);2分图像2例,占7%(2/30);1分图像1例,占3%(1/30);常规3D-SPACE序列得4分图像的15例,占50%(15/30);3分图像9例,占30%(9/30);2分图像5例,占17%(5/30);1分图像1例,占3%(1/30),2种序列间差异有统计学意义(Z=2.254,P=0.024),优化3D-SPACE序列在呼吸运动伪影、胆总管、胆囊管、肝总管、肝内胆管显示,病灶清晰度和图像质量方面均优于常规3D-SPACE序列。结论优化参数3D-SPACE能减少扫描时间,明显减少儿童MRCP的运动伪影,改善图像质量。在儿童磁共振胆道系统疾病检查中具有较高的应用价值。
Objective To explore the application value of the optimized parameter of 3D sampling perfection with application optimized contrasts using different flip angle evolutions(3D-SPACE)in children MRCP examination.Methods MRCP scanning was performed on 30 children with optimized 3d-space sequence and conventional 3d-space sequence.The images obtained from the two scanning sequences were compared and analyzed.The quality differences between the two scans were compared.ResultsThe optimized scanning time of 3d-space sequence was(50±7)s,normal 3D-space sequence scan time was(210±33)s,and the difference was statistically significant(t=15.95,P=0.000).The 4-point of image from the optimized parameter 3D-space sequence was found in 24 cases,accounting for 80%(24/30);3-point in 3 cases,accounting for 10%(3/30);2-point in 2 cases,accounting for 7%(2/30);and 1-point in 1 case,accounting for 3%(1/30).The 4-point of image from the conventional 3 d-space sequence was found in 15 cases,accounting for 50%(15/30);3-point in 9 cases,accounting for 30%(9/30);2-point in 5 cases,accounting for 17%(5/30);and 1-point in 1 case,accounting for 3%(1/30).The difference was statistically significant(Z=2.254,P=0.024).Optimization of 3d-space sequence was superior to conventional 3D-space sequence in terms of respiratory movement artifact,common bile duct,gallbladder duct,common hepatic duct,intrahepatic bile duct display,focal clarity,and image quality.Conclusion Optimizing 3D-SPACE can reduce scanning time,significantly reduce the motion artifacts of MRCP in children,and improve image quality.It has high application value in the examination of children's biliary system diseases by magnetic resonance.
