摘要
目的探讨保留后关节囊在髋关节置换手术中的应用价值。方法选取2017年10月至2019年10月收治的髋关节置换手术患者118例,根据手术方案分为对照组和观察组,每组59例。观察组术中保留后关节囊,对照组术中未保留后关节囊,于术前、术后、随访6个月时行疼痛评分、Harris评分、WOMAC评分、生活质量评分,比较2组患者的手术指征、术后并发症、临床疗效。结果2组患者术后WOMAC评分、疼痛评分较手术前显著降低,2组患者手术后生活质量评分、Harris评分较术前增加(P<0.05)。2组6个月随访疼痛评分、WOMAC评分较术前、术后显著降低;2组患者随访6个月Harris评分、生活质量评分较术前、术后显著增加(P<0.05)。观察组患者术后疼痛评分、WOMAC评分低于对照组;观察组患者术后生活质量评分、Harris评分高于对照组(P<0.05)。观察组6个月随访WOMAC评分、疼痛评分低于对照组;观察组患者随访6个月Harris评分、生活质量评分高于对照组。观察者患者术后住院时间短于对照组,观察组总有效率高于对照组,观察组术后并发症发生率小于对照组(P<0.05)。结论保留后关节囊有助于提高髋关节置换手术患者的临床疗效,可减轻疼痛、加速恢复、提高安全性。
Objective To investigate the application value of posterior joint capsule preservation in hip replacement.Methods A total of 118 patients who underwent hip replacement in our hospital from October 2017 to October 2019 were enrolled in the study.According to operation plan,these patients were divided into control group(n=59)and observation group(n=59).The patients in observation group were treated by preserving posterior joint capsule during operation,however,the patients in control group were treated without preserving posterior joint capsule.The pain score,Harris score,WOMAC score,life quality score before operation,after operation and after 6-month follow up as well as the operation indexes,postoperative complications,clinical effects were observed and compared between the two groups.Results After operation the pain scores and WOMAC scores in both groups were decreased significantly,however,the Harris scores and life quality scores were increased significantly(P<0.05).And the pain scores and WOMAC scores after 6-month follow up in both groups were decreased significantly,but the Harris scores and life quality scores were increased significantly.Moreover the pain scores and WOMAC scores after operation and after 6-month follow up in observation group were significantly lower than those in control group,however,the Harris scores and life quality scores in observation group significantly higher than those in control group(P<0.05).Moreover the postoperative hospitalization time in observation group was significantly shorter than that in control group,however,the total effective rate in observation group was significantly higher than that in control group,moreover,the incidence rate of postoperative complications in observation group was significantly lower than that in control group(P<0.05).Conclusion To reserve the posterior joint capsule is helpful to improve the clinical effects of hip replacement,which can relieve pain,accelerate recovery and improve safety.
