摘要
目的探讨围化疗期自拟愈疡汤联合沙利度胺对急性白血病口腔溃疡的疗效。方法选择2016年11月至2018年4月收治的急性白血病患者88例,按照随机数表法分为观察组和对照组,每组44例。化疗前3 d,对照组给予复合维生素B2片和沙利度胺片口服,观察组在对照组的基础上加用自拟的愈疡汤。2周后,比较2组患者的口腔溃疡发生情况,比较口腔溃疡疼痛缓解时间和愈合时间,观察患者口腔清洁度评分及口腔细菌培养阳性菌结果。结果观察组口腔溃疡总发生率为27.3%显著低于对照组的43.2%,差异有统计学意义(χ^(2)=5.538,P<0.05);观察组口腔溃疡Ⅲ度+Ⅳ度的发生率为4.5%显著低于对照组的13.6%(χ^(2)=5.030,P<0.05);观察组患者口腔溃疡疼痛缓解时间和溃疡愈合时间显著短于对照组(t=2.051、2.218,P<0.05)。治疗第5、10、15天后,观察组患者口腔清洁度评分显著高于对照组(t=1.268、2.063、2.074,P<0.05)。治疗第5、10、15天后,观察组患者口腔细菌培养阳性菌例数占比显著低于对照组,差异有统计学意义(χ^(2)=3.261、3.267、4.298,P<0.05)。结论自拟愈疡汤联合沙利度胺治疗方案可降低急性白血病患者口腔溃疡的发生率,能够有效缩短患者的发病时间,值得应用。
Objective To investigate the preventive and therapeutic effects of self-made Yuyang decoction combined with thalidomide in treatment of acute leukemia oral ulcers during chemotherapy.Methods A total of 88 patients with acute leukemia who were treated in our hospital from November 2016 to April 2018 were enrolled in the study,who were divided into observation group(n=44)and control group(n=44)according to random number table method.At 3 days before chemotherapy,the patients in control group were given multivitamin B2 tablets and thalidomide tablets orally,however,the patients in observation group,on the basis of control group,were given self-made Yuyang decoction.After 2-week medication,the incidence of oral ulcer,and the relief time and healing time of oral ulcer pain as well as the oral cleanliness score and positive bacteria culture were observed and compared between the two groups.Results The total incidence rate of oral ulcer in observation group was 27.3%,which was significantly lower than that(43.2%)in control group(P<0.05).The incidence rate of oral ulcerⅢdegreeⅣin observation group was 4.5%,which was significantly lower than that(13.6%)in control group(P<0.05).The pain relief time and healing time in observation group were significantly shorter than those in control group(P<0.05).Moreover the oral cleanliness scores at 5d,10d,15d after treatment in observation group were significantly higher than those in control group(P<0.05),and the numbers of positive bacteria in oral bacteria culture in observation group(0,2,3)were significantly lower than those(5,8,11)in control group(P<0.05).Conclusion The treatment scheme of self-made Yuyang decoction combined with thalidomide can reduce the incidence rate of oral ulcers in patients with acute leukemia,and shorten the onset time of patients.
