摘要
目的:探讨甲硫咪唑联合糖皮质激素在甲亢患者中的治疗效果及对甲状腺过氧化物酶抗体(TPO-Ab)、抗甲状腺球蛋白抗体(TGAb)及促甲状腺激素受体抗体(TRAb)的影响。方法:选择2018年3月-2019年1月本院甲亢患者120例作为研究对象,采用抽签法分为对照组和观察组,各60例。对照组采用甲硫咪唑治疗,观察组在对照组基础上联合糖皮质激素治疗,两组均完成6个月治疗并完成6个月随访。比较两组甲状腺、内分泌功能、TPO-Ab、TGAb及TRAb水平、药物安全性、复发率。结果:观察组治疗后6个月FT_(3)、FT_(4)、ANP、TT_(3)和TT_(4)水平均低于对照组(P<0.05);观察组治疗后6个月TPO-Ab、TGAb及TRAb水平低于对照组(P<0.05);两组治疗过程中体重增加、白细胞减少、皮疹发生率及治疗后1、3个月复发率无统计意义(P>0.05);观察组治疗后6个月复发率低于对照组(P<0.05)。结论:甲硫咪唑联合糖皮质激素能改善甲亢患者甲状腺、内分泌功能,能降低TPO-Ab、TGAb及TRAb水平,未增加不良反应发生率,能降低治疗后复发率,值得推广应用。
Objective:To investigate the therapeutic effect of Methimazole combined with glucocorticoids in patients with hyperthyroidism and its influence on level of thyroid peroxidase antibody (TPO-Ab),thyroid globulin antibody (TGAb) and thyrotrophin receptor antibody (TRAb).Method:From March 2018 to January 2019,120 patients with hyperthyroidism in our hospital were selected as research subjects,and they were divided into the control group and the observation group by lottery,with 60 cases in each group.The control group was treated with Methimazole,and the observation group was combined with glucocorticoid treatment on the basis of the control group.Both groups completed 6 months of treatment and completed 6 months of follow-up.The thyroid,endocrine function,TPO-Ab,TGAb and TRAb levels,drug safety,recurrence rate of the two groups were compared.Result:The levels of FT_(3),FT_(4),ANP,TT_(3) and TT_(4) in the observation group were lower than those in the control group at 6 months after treatment (P<0.05);the levels of TPO-Ab,TGAb and TRAb in the observation group were lower than those in the control group at 6 months after treatment (P<0.05);there were no significant differences in the incidence of weight gain,leukopenia,rash and recurrence rate 1 and 3 months after treatment of the two groups (P>0.05);the recurrence rate 6 months after treatment in the observation group was lower than of the control group (P<0.05).Conclusion:Methimazole combined with glucocorticoids can improve the thyroid and endocrine functions of patients with hyperthyroidism,reduce the levels of TPO-Ab,TGAb and TRAb,without increasing the incidence of adverse reactions,and can reduce the recurrence rate after treatment.It is worthy of popularization and application.
