摘要
目的观察全膝关节置换术治疗老年重度膝骨性关节炎的临床效果。方法选取2017年5月至2020年3月我院收治的96例老年重度膝骨性关节炎患者,根据治疗方法不同将其分为药物组(48例,药物治疗)和手术组(48例,全膝关节置换术治疗)。比较两组的炎症因子水平、膝关节功能评分、临床疗效及日常生活能力评分。结果治疗后,两组的TNF-α、IL-6、CTX-Ⅰ、CTX-Ⅱ水平及OKS评分均显著降低,HSS及Barthel指数评分均显著升高,且手术组优于药物组(P<0.05)。手术组的治疗总有效率显著高于药物组(P<0.05)。结论采用全膝关节置换术治疗老年重度膝骨性关节炎,能够有效改善患者炎症因子水平,促进其膝关节功能恢复,提高其日常生活能力,效果显著。
Objective To observe the clinical effect of total knee arthroplasty in the treatment of elderly patients with severe knee osteoarthritis.Methods Ninety-six elderly patients with severe knee osteoarthritis treated in our hospital from May 2017 to March 2020 were selected and divided into drug group(48 cases,drug treatment)and operation group(48 cases,total knee arthroplasty treatment)according to different treatment methods.The levels of inflammatory factors,knee function scores,clinical efficacy and ADL scores were compared between the two groups.Results After treatment,the levels of TNF-α,IL-6,CTX-Ⅰ,CTX-Ⅱand OKS scores in both groups significantly decreased,while the scores of HSS and Barthel index significantly increased,and those in the operation group were better than the drug group(P<0.05).The total effective rate of treatment in the operation group was significantly higher than that in the drug group(P<0.05).Conclusion Total knee arthroplasty in the treatment of elderly patients with severe knee osteoarthritis can effectively improve the levels of inflammatory factors,promote the recovery of knee joint function,and improve their ability of daily living,with remarkable curative effect.
