摘要
目的:探究乌司他丁联合银杏达莫注射液对重症脓毒症患者炎性因子及微循环的影响。方法:选取2016年3月~2019年3月于某院接受治疗的120例重症脓毒症患者为研究对象,采用随机数字表法分为对照组和观察组各60例。对照组采用连续性血液净化治疗,观察组在此基础上联合乌司他丁+银杏达莫注射液治疗,共治疗7d,比较两组治疗前及治疗7d时的炎症因子水平[白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、白细胞介素-1β(IL-1β)、肿瘤坏死因子(TNF-α)]及微循环[总血管密度(TVD)、灌注血管密度(PVD)、灌注血管比例(PPV)、微血管流动指数(MFI)]的变化情况。结果:治疗前,两组IL-6、IL-8、IL-1β、TNF-α水平对比,差异无统计学意义(P>0.05);治疗7d后,两组IL-6、IL-8、IL-1β、TNF-α水平均低于治疗前,且观察组炎性因子水平各项指标均显著低于对照组,差异有统计学意义(P<0.05);治疗前,两组的TVD、PVD、PPV、MFI水平比较差异无统计学意义(P>0.05);治疗7d后,两组TVD、PVD水平低于治疗前,PPV、MFI水平高于治疗前,且观察组TVD、PVD水平低于对照组,PPV、MFI水平高于对照组,差异有统计学意义(P<0.05)。结论:乌司他丁联合银杏达莫注射液治疗重症脓毒症患者有显著疗效,利于降低血清炎性因子水平,改善患者微循环,促进患者病情转归。
Objective:To explore the effect of ulinastatin combined with ginkgo leaf extract and dipyridamole injection on inflammatory factors and microcirculation in patients with severe sepsis.Methods:A total of 120 patients with severe sepsis who were treated in a hospital from March 2016 to March 2019 were selected as the research objects.They were divided into 2 groups by random number table method,with 60 cases in each group.The control group was treated with continuous blood purification,and the observation group was treated with ulinastatin+ginkgo leaf extract and dipyridamole injection on this basis for a total of 7 days.The changes of levels of inflammatory factors[IL-6(IL-6),interleukin-8(IL-8),interleukin-1β(IL-1β),tumor necrosis factor(TNF-α)]and microcirculation[total vessel density(TVD),perfusion,blood vessel density(PVD),perfusion vessel ratio(PPV),microvascular flow index(MFI)]before and at 7 days were compared between the two groups.Results:Before treatment,there was no significant difference in the levels of IL-6,IL-8,IL-1β,and TNF-αbetween the two groups(P>0.05).After 7 days of treatment,the IL-6,IL-8,IL-1βand TNF-αlevels in the two groups were lower than those before treatment,and the levels of inflammatory factors in the observation group were significantly lower than those in the control group,the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in TVD,PVD,PPV,and MFI level between the two groups(P>0.05).After 7 days of treatment,the levels of TVD and PVD in the two groups were lower than those before treatment,while the levels of PPV and MFI were higher than those before treatment,the levels of TVD and PVD in the observation group were lower than those in the control group,and the PPV and MFI levels were higher than those the control group,the difference was statistically significant(P<0.05).Conclusion:Ulinastatin combined with ginkgo leaf extract and dipyridamole injection has a significant effect in the treatment of patients with severe sepsis,which is beneficial to reduce the level of serum inflammatory factors,improve the microcirculation of patients,and promote the outcome of patients.
