摘要
目的观察不同牵引方案下行股骨近端防旋髓内钉(PFNA)内固定治疗老年股骨粗隆间骨折(IF)的效果。方法抽取2018年5月至2020年5月河南医学高等专科学校附属医院收治的19例采用手法牵引的老年IF患者作为观察组,抽取同期21例采用牵引床牵引的老年IF患者作为对照组,两组均接受PFNA治疗。比较两组围术期指标;记录两组住院期间并发症发生情况;随访12个月,记录两组康复情况,即骨折愈合时间及不同时点(术后1、3、6个月)髋关节功能。结果观察组摆放体位至切皮时间、手术时间短于对照组(P<0.05);两组髋内翻、骨折延迟愈合及压疮发生率相近(P>0.05);两组骨折愈合时间相近(P>0.05);两组术后3、6个月的Harris髋关节功能评分较术后1个月升高(P<0.05)。结论不同牵引方案下行PFNA内固定治疗老年IF均有一定效果,手法牵引可缩短患者摆放体位至切皮时间、手术时间,有助于提高手术效率,且患者术后并发症、骨折愈合及髋关节功能恢复未受明显影响。
Objective To compare the effect of proximal femoral nail antirotation(PFNA)internal fixation under different traction scheme in the treatment of elderly intertrochanteric fracture(IF).Methods Nineteen elderly IF patients with manual traction in the Affiliated Hospital of Henan Medical College from May 2018 to May 2020 were selected as the observation group,and 21 elderly IF patients with traction bed traction in the same period were selected as the control group,both groups were treated with PFNA.The perioperative indexes were compared between the two groups;the occurrence of complications during hospitalization between the two groups was recorded;all patients were followed up for 12 months,the recovery of the two groups was recorded:the fracture healing time and hip joint function at different time points(1 month,3 months and 6 months after operation).Results The time from body position to skin incision and operation time in the observation group were shorter than those in the control group(P<0.05);the incidence of hip varus,delayed union of fracture and pressure ulcer were similar in both groups(P>0.05);the fracture healing time in both groups was similar(P>0.05);the Harris hip function scores 3 and 6 months after operation were increased compared with those 1 month after operation(P<0.05).Conclusions PFNA internal fixation under different traction schemes has certain effect in the treatment of elderly IF,manual traction can shorten the time from body position to skin incision and operation time,which is helpful to improve the operation efficiency,and postoperative complications,fracture healing and hip function recovery are not significantly affected.
