摘要
目的探讨分析脐血胆红素联合溶血三项检查对新生儿ABO溶血病及高胆红素血症的预测价值。方法方便选取2018年8月—2019年8月该院产科出生的180名O型血母亲的足月健康新生儿为研究对象,根据母婴血型分为母婴血型相同组、母婴血型不同组两组,根据脐血胆红素水平分成5组,记录血清、血型检测结果,分析脐血胆红素、溶血三项与ABO溶血病及高胆红素血症的关系。结果母婴血型不同组中HDN发生率(44.66%)及新生儿高胆红素血症发生率(31.06%)均显著高于母婴血型相同组(15.58%、11.68%),差异有统计学意义(χ^(2)=17.056、9.408,P<0.05)。脐血胆红素水平预测新生儿高胆红素血症特异性较强均≥81.63%,脐血胆红素0~21μmol/L和≥55μmol/L时特异性为100.00%,溶血三项不同模式下脐血胆红素水平差异有统计学意义(F=107.490,P<0.05),新生儿高胆红素血症发生率差异有统计学意义(H=18.921,P<0.05)。结论母亲为O型血的新生儿存在血型不合应进行脐血胆红素及溶血三项检测,可早期预测ABO溶血症及高胆红素血症的发生率。
Objective To explore and analyze the predictive value of the three tests of cord blood bilirubin combined with hemolysis for neonatal ABO hemolytic disease and hyperbilirubinemia.Methods 180 full-term healthy newborns of type O mothers born from August 2018 to August 2019 to the hospital's obstetrics department were conveniently selected as the research objects.According to the blood types of mothers and infants,they were divided into groups of the same blood type of mothers and infants,and different blood types of mothers and infants.The two groups were divided into five groups according to the cord blood bilirubin level.The serum and blood type test results were recorded,and the relationship between cord blood bilirubin and hemolysis and ABO hemolytic disease and hyperbilirubinemia was analyzed.Results The incidence of HDN(44.66%)and neonatal hyperbilirubinemia(31.06%)in different groups of maternal and infant blood types were significantly higher than those of the same group(15.58%,11.68%),the difference was statistically significant(χ^(2)=17.056,9.408,P<0.05),the strong specificity of cord blood bilirubin levels in predicting neonatal hyperbilirubinemia were all≥81.63%,cord blood bilirubin 0~21μmol/L and≥55μmol/L was 100.00%,the cord blood bilirubin level was statistically different under the three different modes of hemolysis,the difference was statistically significant(F=107.490,P<0.05),and the incidence of neonatal hyperbilirubinemia was statistically different,the difference was statistically significant(H=18.921,P<0.05).Conclusion Newborns with O-type blood should be tested for cord blood bilirubin and hemolysis,which can predict the incidence of ABO hemolysis and hyperbilirubinemia early.
