摘要
目的:对比研究拮抗剂与微刺激两种促排卵方案治疗高龄不孕女性的临床效果,为临床提供一定的理论参考。方法:选取235例高龄不孕患者视为研究对象,经数字随机表法分为拮抗剂组100例和微刺激组135例。拮抗剂组采用拮抗剂促排卵方案,微刺激组采用微刺激促排卵方案,比较两组促排卵效果和临床妊娠结局。结果:在Gn使用时间、Gn用量方面,微刺激组均显著少于拮抗剂组,但获卵数、优质胚胎数两组差异无统计学意义(P>0.05)。在周期取消率方面,拮抗剂组明显低于微刺激组,差异有统计学意义(P<0.05),两组临床妊娠率、早产率、异位妊娠率比较,差异无统计学意义(P>0.05)。结论:拮抗剂与微刺激是两种治疗高龄不孕女性的常见促排卵方案,二者有着各自的特点和优势。拮抗剂促排卵方案周期取消率低,微刺激促排卵方案Gn量相关指标更好,建议临床根据具体情况个体化选择促排卵方案。
Objective To compare and study the clinical effect of antagonist and micro-stimulation for ovulation induction in the treatment of elderly infertile women,so as to provide a theoretical reference for the clinic.Method A total of 235 infertile elderly patients admitted to the department of Reproduction of our hospital were selected as study subjects,and were divided into antagonists group 100 cases and Micro stimulation group 135 cases by numerical random table method.antagonists group adopted the antagonist ovulation induction regimen,Micro stimulation group adopted the micro-stimulation ovulation induction regimen,compared the ovulation induction effect and clinical pregnancy outcomes of the two groups.Results In terms of Gn usage time and Gn dosage,Micro stimulation group was significantly lower than antagonists group,but there was no statistically significant difference between the two groups in the number of eggs and the number of high-quality embryos(P>0.05).In terms of cycle cancellation rate,antagonists group was significantly lower than Micro stimulation group(P<0.05),and there was no significant difference in clinical pregnancy rate,premature birth rate and ectopic pregnancy rate between the two groups(P>0.05).Conclusion Antagonist and microstimulation are two common ovulation induction schemes for elderly infertile women,which have their own characteristics and advantages.The cycle cancellation rate of the antagonist ovulation induction scheme is low,and Gn volume related indicators of the micro-stimulation ovulation induction scheme are better.Therefore,it is suggested to select the ovulation induction scheme according to the specific situation.
