长效胰岛素与口服降糖药联合方案在治疗2型糖尿病的疗效与糖化血红蛋白和尿微量蛋白水平分析 被引量:16

摘要 目的探讨在2型糖尿病患者治疗中联合予以长效胰岛素与口服降糖药的疗效以及对其糖化血红蛋白和尿微量蛋白水平的影响。方法研究对象以2019年6月—2020年6月来该院接受诊治的142例2型糖尿病患者进行研究,以随机数表法作为分组依据,均分为常规组(予以口服降糖药治疗)与实验组(联合口服降糖药与长效胰岛素治疗),比较两组临床疗效。血糖控制情况,治疗干预前后糖化血红蛋白、尿微量蛋白水平变化情况及用药不良反应发生率。结果实验组临床治疗总有效率(84.5%)显著高于常规组(97.2%),差异有统计学意义(χ^(2)=6.859,P<0.05);实验组空腹血糖水平与餐后2 h血糖水平显著低于常规组,差异有统计学意义(t=5.358、8.151,P<0.05);实验组糖化血红蛋白水平低于常规组,尿微量蛋白则高于常规组,差异有统计学意义(t=6.581、6.533,P<0.05);实验组用药治疗不良反应发生率(2.8%)显著低于常规组(14.0%),差异有统计学意义(χ^(2)=5.826,P<0.05)。结论在治疗糖尿病患者时,相较于单一应用口服降糖药物方案治疗,联合口服降糖药与长效胰岛素,疗效更为确切,不仅能够控制患者血糖水平,而且具有较高安全性,建议在临床治疗中推广与应用。 Objective To investigate the efficacy of combined long-acting insulin and oral hypoglycemic agents in the treatment of patients with type 2 diabetes,as well as its effect on glycosylated hemoglobin and urinary microprotein levels.Methods The study subjects were 142 patients with type 2 diabetes who came to the hospital for diagnosis and treatment from June 2019 to June 2020.Based on the random number table method,they were divided into the conventional group(treated with oral hypoglycemic drugs)and the experimental group(combined oral administration antidiabetic drugs and long-acting insulin therapy),compare the clinical efficacy of the two groups,boold sugar control changes in glycosylated hemoglobin,urinary microprotein levels before and after treatment intervention,and the incidence of adverse drug reactions.Results The total effective rate of clinical treatment in the experimental group(84.5%)was significantly higher than that of the conventional group(97.2%),the difference was statistically significant(χ^(2)=6.859,P<0.05);the fasting blood glucose level and the blood glucose level 2 h after a meal in the experimental group were significantly lower than those of the conventional group,the difference was statistically significant(t=5.358,8.151,P<0.05);the level of glycosylated hemoglobin in the experimental group was lower than that of the conventional group,and the urinary microprotein was higher than that of the conventional group,the difference was statistically significant(t=6.581,6.533,P<0.05);the incidence of adverse drug reactions in the experimental group(2.8%)was significantly lower than the conventional group(14.0%),the difference was statistically significant(χ^(2)=5.826,P<0.05).Conclusion In the treatment of diabetic patients,compared with the single application of oral hypoglycemic drugs,the combination of oral hypoglycemic drugs and long-acting insulin had a more accurate effect.It can not only control the patient's blood glucose level,but also had higher safety.It is recommended for promotion and application in clinical treatment.
