摘要
目的:探讨整形美容手术中右美托咪定的应用对患者心脑血管系统的影响。方法:选取2019年1月-2020年5月在笔者医院进行整形美容手术178例患者为研究对象,随机数字法分为两组,两组在术中均实施局部麻醉,两组均静脉推注1μg/kg的芬太尼,局部麻醉药物为浓度0.5%的利多卡因、1:100000的肾上腺素,对照组联合应用生理盐水,研究组联合应用右美托咪定,对比右美托咪定的应用对术中高血压、心动过速发生的影响。结果:两组手术时间比较差异无统计学意义(P>0.05)。但研究组的芬太尼、咪达唑仑用量均少于对照组,差异有统计学意义(P<0.05)。对照组T2、T3时间的HR快于研究组,RR慢于研究组,MAP高于研究组,差异有统计学意义(P<0.05)。两组低血压、心动过缓的发生率比较差异无统计学意义(P>0.05)。但研究组高血压、心动过速及呼吸抑制的发生率低于对照组,差异有统计学意义(P<0.05)。研究组苏醒时间、清醒时间均短于对照组,差异有统计学意义(P<0.05)。结论:在整形美容手术中,应用右美托咪定能够更好地稳定患者的心率及血压,防止其过度波动引起不良事件的发生,值得临床应用。
Objective To investigate the effect of dexmedetomidine on cardiovascular and cerebrovascular system in plastic surgery.Methods From January 2019 to May 2020,178 patients undergoing plastic and aesthetic surgery in our hospital were selected as the study objects.Randomly divided into two groups,the two groups were given local anesthesia during the operation,both groups were given intravenous injection of 1μg/kg fentanyl,the local anesthetic was 0.5%lidocaine and 1:100000 epinephrine.The control group was given normal saline,the study group was given dexmedetomidine.And the effects of dexmedetomidine on intraoperative hypertension and tachycardia were compared.Results There was no significant difference in operation time between the two groups(P>0.05).But the dosage of fentanyl and midazolam in the study group was less than those in the control group,the differences were statistically significant(P<0.05).HR at T2 and T3 in the control group were faster than those in the study group,RR were slower than those in the study group,and MAP were higher than those in the study group(P<0.05).There was no significant difference in the incidence of hypotension and bradycardia between the two groups(P>0.05).But the incidence of hypertension,tachycardia and respiratory depression in the study group were lower than those in the control group(P<0.05).The recovery time and awake time in the study group were shorter than those in the control group(P<0.05).Conclusion In plastic surgery,dexmedetomidine can better stabilize the heart rate and blood pressure of patients,prevent the occurrence of adverse events caused by excessive fluctuations,which is worthy of clinical application.
