乳腺癌组织中miR218-5p、IGF-1的表达水平及其临床意义 被引量:5

摘要 目的探讨乳腺癌组织中微小RNA-218-5p(miR218-5p)、胰岛素样生长因子-1(IGF-1)表达水平的变化及其临床意义。方法选取中国科学院大学深圳医院(光明)甲乳外科2016年6月至2019年8月经手术切除的120例乳腺癌组织蜡块标本作为乳腺癌组,另随机抽取60例乳腺癌患者标本切缘的癌旁正常组织(距癌旁组织的距离超过5 cm)作为癌旁组,并选择我院同期住院治疗的90例良性乳腺纤维瘤病变组织作为良性病变组。比较三组组织标本miR218-5p、IGF-1水平的表达,比较乳腺癌组不同TNM分期患者组织标本miR218-5p、IGF-1水平的表达,采用Spearman相关性分析法分析miR218-5p、IGF-1与TNM分期的相关性。结果癌旁组组织中miR218-5p、IGF-1的阳性细胞表达率分别为(1.21±0.28)%、(2.63±0.20)%,良性病变组分别为(0.70±0.11)%、(3.80±0.55)%,乳腺癌组分别为(0.44±0.09)%、(6.51±1.10)%,乳腺癌组织中miR218-5p的阳性细胞表达率明显低于良性病变组和癌旁组,IGF-1的阳性细胞表达率明显高于良性病变组和癌旁组,良性病变组组织中miR218-5p阳性细胞表达率明显低于癌旁组,IGF-1的阳性细胞表达率明显高于癌旁组,差异均有统计学意义(P<0.05);Ⅰ级和Ⅱ级乳腺癌组织中的miR218-5p、IGF-1的阳性细胞表达率分别为(0.35±0.03)%、(6.90±1.34)%,Ⅲ级和Ⅳ级分别为(0.23±0.01)%、(11.85±2.12)%,Ⅲ级和Ⅳ级乳腺癌组织中的的miR218-5p阳性细胞率明显低于Ⅰ级和Ⅱ级乳腺癌,IGF-1阳性细胞率明显高于Ⅰ级和Ⅱ级乳腺癌,差异均有统计学意义(P<0.05);经Spearman相关性分析结果显示,miR218-5p与TNM分期呈负相关(r=-0.385,P<0.05),IGF-1与TNM分期呈正相关(r=0.693,P<0.05),乳腺癌组织中miR218-5p与IGF-1的表达呈负相关(r=-0.326,P<0.05)。结论乳腺癌组织中,miR218-5p表达较低,IGF-1有明显高表达,miR218-5p和IGF-1表达水平与临床病理特征有明显相关性。 Objective To study the expression levels of microRNA-218-5p(miR218-5p)and insulin-like growth factor-1(IGF-1)in breast cancer tissues and their clinical significance.Methods A retrospective analysis was performed on 120 cases of breast cancer specimens,60 cases of paracancerous tissue specimens(with distance from the paracancerous tissue of more than 5 cm),and 90 cases of benign breast fibroma in Department of Thyroid and Breast Surgery,Shenzhen Hospital(Guangming),University of Chinese Academy of Sciences from June 2016 to August 2019.The expression of miR218-5p and IGF-1 were compared among the three groups,and also among different TNM stages of breast cancer.The correlation between miR218-5p,IGF-1 and TNM stages was analyzed by Spearman correlation analysis.Results The positive expression rates of miR218-5p and IGF-1 were(1.21±0.28)%and(2.63±0.20)%in the paracancerous tissue group,(0.70±0.11)%and(3.80±0.55)%in the benign lesion group,(0.44±0.09)%and(6.51±1.10)%in the breast cancer group;the positive expression rate of miR218-5p in breast cancer group were significantly lower than that in benign lesion group and paracancerous tissue group;the expression rate of IGF-1 were significantly higher than that in benign lesion group and paracancerous tissue group;the positive expression rate of miR218-5p in benign lesion group was significantly lower than that in paracancerous tissue group,and the positive expression rate of IGF-1 in benign lesion group was significantly higher than that in paracancerous tissue group;the differences were all statistically significant(P<0.05).The positive expression rates of miR218-5p and IGF-1 were(0.35±0.03)%and(6.90±1.34)%in gradeⅠand gradeⅡbreast cancers,(0.23±0.01)%and(11.85±2.12)%in gradeⅢandⅣbreast cancers;the rate of miR218-5p in gradeⅢand gradeⅣbreast cancer was significantly lower than that of gradeⅠandⅡbreast cancer,and the rate of IGF-1 was significantly higher than that of gradeⅠandⅡbreast cancer;the differences were statistically significant(P<0.05).Spearman correlation analysis showed that miR218-5p was negatively correlated with TNM staging(r=-0.385,P<0.05),IGF-1 was positively correlated with TNM staging(r=0.693,P<0.05),and the expression of miR218-5p and IGF-1 were negatively correlated in breast cancer(r=-0.326,P<0.05).Conclusion In patients with breast cancer,the expression of miR218-5p was low and IGF-1 was highly expressed,and the changes of miR218-5p and IGF-1 levels were significantly correlated with clinicopathological features,which provide a certain basis for the diagnosis and screening of breast cancer.
