摘要
介绍刘莉教授以“治未病”思想防治代谢综合征(metabolic syndrome,MS)的经验,从“未病先防、既病防变、瘥后防复”3个方面予以阐述。刘莉教授强调代谢综合征的发病是内、外因相互作用的结果,内因为先天禀赋不足,脾肾亏虚,外因为饮食不节、劳逸失度、情志失调等。痰湿是MS的病理因素,贯穿于发病的全过程,病机特点为虚实夹杂,脾肾亏虚为本,痰瘀热互结为标。在MS痰浊内生期注重“未病先防”,通过中医健康宣教以及对饮食、运动、情志的调理以防病于未然;在MS痰瘀互结期重视“既病防变”,以健脾补肾,祛痰瘀热邪之标实为治则,预防变证发生;MS“瘥后”期的调摄从心理、生活和药物几个方面进行,以防病复发。
The purpose of this study is to introduce Professor Liu Li’s experience in the prevention and treatment of metabolic syndrome(MS)with the idea of“preventive treatment of disease”.It is elaborated from the three aspects of“prevention before a disease,prevention of a change after the disease,prevention of a recurrence after a recovery”.Professor Liu Li stresses that the onset of metabolic syndrome is the result of the interaction of internal and external factors,namely internally due to the lack of congenital endowment,the deficiency of the spleen and kidney,externally due to improper diet,imbalance of work and leisure,and emotional disorders,etc.Phlegm dampness is the pathological factor of MS,which runs through the whole process of the disease.Its pathogenesis is characterized by the mixture of deficiency and excess,with the deficiency of the spleen and kidney as the root,and the mutual intermingling of phlegm,blood stasis and heat as its manifestation.In the endogenetic phase of MS phlegm and turbidity,“prevention before a disease”should be paid attention to through performing the TCM health education and regulating the diet,exercises,and emotions.In the phase of the mutual intermingling of MS phlegm and blood stasis,“prevention of a change after the existing disease”is emphasized by taking invigorating the spleen and tonifying the kidney to eliminate phlegm,blood stasis,and heat evils as the treatment principle,preventing the occurrence of a syndrome change.In the MS stage of prevention of a recurrence after the recovery,health preservation should be carried out from the aspects of psychology,lifestyle and medicine to prevent the recurrence of the disease.
