摘要
目的分析模块化新型工具用于单次长时间俯卧位通气的急性呼吸窘迫综合征(ARDS)患者中的价值,观察患者压力性损伤发生情况,旨在为未来俯卧位通气治疗的安全展开提供参考。方法选择我院2018年4月-2020年4月收治的ARDS患者60例作为研究对象,根据随机数字表法将患者分为对照组和观察组,各30例。两组患者均接受单次至少12 h的俯卧位通气,两组俯卧位通气标准流程一致,对照组采用垫软枕方式,观察组使用模块化新型工具。比较两组患者俯卧位通气期间压力性损伤发生情况、入住ICU时间、机械通气时间、死亡率、并发症发生情况;分别于俯卧位前、俯卧位2h、俯卧位10h比较两组患者氧合情况。结果观察组面部、胸部压力性损伤发生率低于对照组,差异有统计学意义(P<0.05);组间其他部位压力性损伤发生率比较差异无统计学意义(P>0.05)。两组患者压力性损伤以Ⅰ~Ⅱ期为主,观察组压力性损伤总发生率低于对照组,差异有统计学意义(P<0.05)。结论模块化新型工具用于ARDS患者单次长时间俯卧位通气中,较传统软垫的使用更利于减少患者俯卧位通气期间压力性损伤的发生。
Objective To analyze the value of modular new tools in acute respiratory distress syndrome(ARDS)patients with single long-time prone position ventilation,to observe the incidence of pressure injury and to provide reference for the safety of prone position ventilation treatment.Methods 60 patients with ARDS in the hospital from April 2018 to April 2020 in Dazu district people’s hospital of Chongqing were selected as subjects.They were divided into control group and experimental group by random number table,with 30 cases in each group.Two groups received single prone position ventilation for 12 h at least.The standard procedure for prone position ventilation in two groups was the same.The difference was that the two groups used different backing tools.The experimental group used modular new tools,and the control group used soft pillow.The pressure injury during prone position ventilation,ICU stays,mechanical ventilation time,mortality and complications were evaluated and compared between two groups;Before the prone position,after the prone position for 2 h and 10h,the oxygenation of the two groups was detected and compared,and the indicators included arterial partial pressure of oxygen(PaO 2),partial pressure of carbon dioxide(PaCO 2),oxygen saturation(SpO 2),oxygenation index(PaO 2/FiO 2).Results The incidence of facial and chest pressure injury in experimental group was lower than that in control group(P<0.05);There was no statistical difference in the incidence of other parts of pressure injury between two groups(P>0.05);The pressure injury of the two groups was mainly in stageⅠ~Ⅱ,the total incidence of pressure injury in the experimental group was lower than that in the control group(P<0.05).Conclusion Compared with traditional soft pillow,the application of modular new tools in ARDS patients with single long-time prone position ventilation can reduce the incidence of pressure injury.
