CDFI对单侧大脑中动脉M1段慢性闭塞后血流动力学的评估分析

摘要 目的探讨彩色多普勒血流成像(CDFI)技术在单侧大脑中动脉(MCA)M1段慢性闭塞后血流动力学评估方面的应用价值。方法收集2018年1月至2021年2月本院脑病科及康复科收治的单侧MCA M1段慢性闭塞患者26例,男14例,女12例,年龄(54.3±11.5)岁;同时选择同期健康志愿者26例作为健康对照组,男13例,女13例,年龄(53.1±10.8)岁。患者均行CDFI及MRI[三维时间飞跃法磁共振血管成像(3D-TOF-MRA)及三维动脉自旋标记(3D-ASL)序列]检查,分析入组患者CDFI所测闭塞侧颈内动脉血管内径、收缩期峰值流速、舒张末期流速、阻力指数及血流量差异,并将闭塞组前后循环脑血流量、全脑血流量与健康对照组进行比较。结果闭塞组患者闭塞侧颈内动脉血流量、舒张末期流速、血管内径分别为(201.15±52.43)ml/min、(16.87±4.46)cm/s、(0.50±0.08)cm,均低于对侧颈内动脉的(260.36±41.22)ml/min、(20.86±4.81)cm/s、(0.55±0.07)cm,差异均有统计学意义(均P<0.05);闭塞组患者闭塞侧颈内动脉与对侧颈内动脉收缩期峰值流速、阻力指数比较,差异均无统计学意义(均P>0.05)。闭塞组患者全脑血流量、前循环脑血流量分别为(672.82±83.29)ml/min、(460.37±80.17)ml/min,均低于健康对照组(729.29±62.58)ml/min、(567.68±61.36)ml/min,后循环脑血流量为(216.57±35.72)ml/min,高于健康对照组(164.73±25.65)ml/min,差异均有统计学意义(均P<0.05)。结论CDFI技术通过脑血流量动力学数值测量和分析,能够精准评估单侧MCA M1段慢性闭塞后脑灌注状态及侧支循环建立。 Objective To investigate the application value of color Doppler flow imaging(CDFI)technique in hemodynamics evaluation after unilateral middle cerebral artery(MCA)M1 segment chronic occlusion.Methods From January 2018 to February 2021,26 patients with unilateral MCA M1 segment chronic occlusion in the departments of encephalopathy and rehabilitation of our hospital were selected,including 14 males and 12 females,with the age of(54.3±11.5)years old;26 healthy volunteers were selected as the healthy control group,including 13 males and 13 females,with the age of(53.1±10.8)years old.All patients underwent CDFI and MRI[three-dimensional time-of-flight magnetic resonance angiography(3D-TOF MRA)and three-dimensional arterial spin labeling(3D-ASL)].The internal carotid artery diameter,peak systolic velocity,end-diastolic velocity,resistance index,and cerebral blood flow of the occluded side measured by CDFI were analyzed,and the circulating blood flow and cerebral blood flow of the occluded group were compared with those of the healthy control group.Results In the occluded group,the blood flow,end-diastolic velocity,and internal diameter of the occluded internal carotid artery were(201.15±52.43)ml/min,(16.87±4.46)cm/s,and(0.50±0.08)cm,which were lower than those of the contralateral internal carotid artery[(260.36±41.22)ml/min,(20.86±4.81)cm/s,and(0.55±0.07)cm](all P<0.05);there were no statistically significant differences in the peak systolic velocity and resistance index(both P>0.05).The cerebral blood flow and anterior circulation cerebral blood flow of the occluded group were(672.82±83.29)ml/min and(460.37±80.17)ml/min,which were lower than those of the healthy control group[(729.29±62.58)ml/min and(567.68±61.36)ml/min],the posterior circulation cerebral blood flow of the occluded group was higher than that of the healthy control group[(216.57±35.72)ml/min vs.(164.73±25.65)ml/min](all P<0.05).Conclusion CDFI can accurately evaluate cerebral perfusion and collateral circulation after unilateral MCA M1 segment chronic occlusion by measuring and analyzing the cerebral blood flow dynamics.
