摘要
目的探讨视网膜脱离术后硅油填充眼复发性视网膜脱离行堤坝式光凝术后取出硅油的临床效果。方法回顾性分析郑州大学第一附属医院2018年9月至2020年6月行玻璃体切除硅油填充术后出现下方复发性视网膜脱离者41例(41只眼)的临床资料。所有患者在硅油取出术前于视网膜下积液区域后缘行堤坝式激光光凝术1~3次,末次光凝术后1周行硅油取出术。随访3个月,观察术后视力及视网膜情况。结果硅油取出术后1个月1只眼视网膜下积液消失,视网膜复位;所有患者硅油取出术后3个月视网膜脱离范围均未扩大,2只眼视网膜下积液消失,视网膜复位。硅油取出术后1个月视力(BCVA)无改变(t=1.846,P=0.075),硅油取出术后3个月视力明显优于术前(t=5.964,P<0.001)。结论硅油填充术后复发性视网膜脱离先行堤坝式光凝术后再进行硅油取出能稳定视网膜脱离范围,效果可靠。
Objective To investigate the clinical efficacy of silicone oil removal after dam-type photocoagulation for silicone oil-tamponaded eyes with recurrent retinal detachment.Methods The clinical data of 41 eyes of 41 cases with inferior recurrent retinal detachment after vitrectomy combined with silicone oil tamponade from Sep.2018 to Jun.2020 in the First Affiliated Hospital of Zhengzhou University were analyzed retrospectively.All cases underwent dam-type laser photocoagulation before the silicone oil removal for 1 to 3 times at the posterior margin of the subretinal fluid area,and silicone oil removal was performed at 1 week after the last laser photocoagulation.The follow-up time was 3 months.The condition of the retina and visual acuity after the operation were observed.Results The retinal detachment area of 41 eyes did not expand at 3 months after the silicone oil removal.The subretinal fluid disappeared and the retina was reattached in 1 eye at 1 month after silicone oil removal.The subretinal fluid disappeared and the retina was reattached in 2 eyes at 3 months postoperatively.The visual acuity at 1 month after silicone oil removal was similar to that before surgery(t=1.846,P=0.075).The visual acuity at 3 months after silicone oil removal was better than that before surgery(t=5.964,P<0.001).Conclusion Recurrent retinal detachment after silicone oil filling is mostly localized in the inferior retina.The detachment range is limited and relatively stable.And dam-type photocoagulation can stabilize the retinal detachment range.The silicone oil can be taken out directly after the dam-type retinal photocoagulation.This method is safe and acceptable by patients.
