摘要
目的探究多层螺旋CT灌注成像扫描对乳腺肿块的诊断价值。方法选取河北北方学院附属第一医院2019年1月至2020年1月期间120例乳腺肿块性病变患者,经多层螺旋CT灌注扫描,其中,乳腺癌63例,良性肿块57例。测量分析血流量(BF)、血容量(BV)、平均通过时间(MTT)、血管表面通透性(PS)等动态图像数据,依据乳腺癌与良性肿块划分不同组别,实施统计学分析。结果乳腺癌组患者的BF、BV、PS均高于良性肿块组,两组差异有统计学意义(P<0.05);两组患者的MTT差异无统计学意义(P>0.05);乳腺癌患者的毛细血管数量增多,管径较正常宽,使微血管容量增加,血流加快。良性肿块患者的微血管密度与乳腺癌实质相似或轻度增多,微血管管径与正常乳腺微血管管径相同,灌注的微血管与非灌注的微血管比例增加不明显。结论在乳腺肿块诊断中实施多层螺旋CT灌注成像扫描,准确率高,利于鉴别、诊断乳腺癌。
Objective To explore the diagnostic value of multi-slice spiral CT perfusion imaging in the diagnosis of breast masses.Methods 120 patients with breast mass lesions in our hospital from January 2019 to January 2020 were selected.Among them,63 cases were breast cancer and 57 cases were benign tumor.Blood flow(BF),blood volume(BV),mean transit time(MTT),vascular surface permeability(PS)and other dynamic image data were measured and analyzed.Results The BF,BV,PS of breast cancer group were higher than that of benign tumor group,the difference was significant(P<0.05);the MTT of two groups was not significant(P>0.05);the number of capillaries in breast cancer patients increased,the diameter was wider than normal,which increased the microvessel volume and blood flow.The microvessel density of patients with benign tumor was similar to that of breast cancer or slightly increased.The diameter of microvessel was the same as that of normal breast cancer.The proportion of perfusion microvessels to non perfusion microvessels was not significantly increased.Conclusion The application of multi-slice spiral CT perfusion imaging in the diagnosis of breast masses has high accuracy,which is use for the differential diagnosis of breast cancer.
