摘要
目的:探讨阿法骨化醇预防早产儿佝偻病疗效及对钙代谢指标影响。方法:选取2017年1月—2020年1月郑州大学第二附属医院儿童保健科收治的符合纳入标准的120例早产儿作为研究对象,依照分层随机数表法分为观察组(n=73)、对照组(n=47)。对照组给予维生素D滴剂治疗,观察组在对照组用药的基础上给予阿法骨化醇治疗,两组患儿均持续用药1个月,对比两组患儿治疗后的佝偻病发生率、不良反应发生率及治疗前后的钙代谢指标,包括血清钙离子(Ca^(2+))、25-羟维生素D3(25OHD3)、骨钙素(BGP)的水平变化。结果:观察组早产儿佝偻病的发生率低于对照组(5.48%vs 19.15%,P<0.05);观察组与对照组早产儿不良反应发生率比较无显著差异(8.22%vs6.38%,P>0.05);治疗后两组早产儿的血清Ca^(2+)、25OHD3、BGP水平检查结果均有所改善(P<0.05),且观察组早产儿上述血清生化指标改善情况均明显优于对照组(P<0.05)。结论:阿法骨化醇可有效预防早产儿佝偻病的发生,对促进骨质形成有重要的临床意义。
Objective:To explore the efficacy of alfacalcidol in preventing rickets in premature infants and its influence on calcium metabolism indexes.Methods:A total of 120 premature infants who met the inclusion criteria and were admitted to the hospital from January 2017 to January 2020 were selected as the research objects.They were divided into observation group(n=73)and control group(n=47)according to the stratified random number table method.The control group was treated with vitamin D drops,and the observation group was treated with alfacalcidol on the basis of the control group.Both groups of children continued to take the drug for 1 months.The incidence of rickets,adverse reactions,incidence and calcium metabolism indicators including changes in serum calcium ion(Ca^(2+)),25-hydroxyvitamin D3(25OHD3),and osteocalcin(BGP)levels before and after treatment were compared between the two groups after treatment.Results:The incidence of rickets in premature infants in the observation group was lower than that in the control group(5.48%vs.19.15%,P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the observation group and the control group(8.22%vs.6.38%,P>0.05).After treatment,the examination results of serum Ca^(2+),25OHD3 and BGP levels of premature infants in the two groups improved(P<0.05),and the improvement of the above-mentioned serum biochemical indicators of premature infants in the observation group was significantly better than those in the control group(P<0.05).Conclusion:Alfacalcidol can effectively prevent the occurrence of rickets in premature infants and has important clinical significance in promoting bone formation.
